刮吸解剖法在直视微创胆道手术中的应用

摘　　要：目的将刮吸解剖法应用于直视微创胆道外科手术中，进一步完善直视微创胆道手术技术，最终达到微创疗效。方法用彭氏多功能手术解剖器联合直视微创手术器械来施行电切、电凝、钝性分离（推剥、刮碎、刮爬等）、同步吸引等操作，完成直视微创胆道外科手术的操作。结果本组132例胆囊结石、胆囊息肉、急性化脓性胆囊炎，胆总管结石，化脓性胆管炎的直视微创胆道手术治疗均在刮吸解剖法的协助下完成，全组无胆道损伤、术后出血及死亡病例，术后无痛或微痛。结论刮吸解剖法结合直视微创技术使直视微创胆道手术更为便捷、精细、准确、安全。Objective Using PMOD in direct-view minimally invasive biliary-tract operation,to consummate its minimally invasive effect. Methods By means of PMOD and direct-view minimally invasive biliary-tract surgical instruments to accomplish the electric incide, electric coagulation, blunt dissection, thus to accomplish the direct-view minimally invasive biliary-tract operation. Results In 132 patients with chole-cystolithiasis, polypus of gall bladder, acute suppurative cholecystitis, common duct stones, suppurative cholangitis, the operations were all accomplished with the help of PMOD. Full set do not have biliary tract injury,blood loss,any ache and death after operation were found. Conclusions PMOD Combined with direct-view minimally invasive biliary-tract skill makes it more fine, accurate, and easy.
