摘 要:目的研究低频复合生理频率慢性电刺激后肺气肿兔膈肌肌浆网(SR)Ca2+-ATP酶及钙离子摄取和释放动力学的适应性变化。方法采用木瓜蛋白酶雾化吸入法建立肺气肿模型:测定正常对照组、肺气肿组和肺气肿慢性电刺激(CES)组SRCa2+-ATP酶的活性及Ca2+的摄取和释放动力学变化。结果(1)(10+40)Hz慢性电刺激后,膈肌SRCa2+-ATP酶活性增高(P<0.05)。10Hz组Ca2+-ATP酶活性降低(P<0.05)。(2)(10+40)Hz慢性电刺激后,膈肌SRCa2+摄取和释放功能增强(P<0.05);10Hz组膈肌SRCa2+摄取和释放功能减弱(P<0.05)。结论不同频率的CES可导致膈肌SRCa2+-ATP酶的活性及Ca2+的摄取和释放动力学产生不同的适应性变化,慢性低频复合生理频率电刺激能增强膈肌SRCa2+-ATP酶活性,提高肺气肿兔膈肌SRCa2+摄取和释放能力,可能是体外膈肌起搏对慢性阻塞性肺疾病(chronic obstructive pulmonary disease,COPD)患者膈肌康复治疗的较好频率模式之一。Objective To investigate the effect of chronic low frequency electrical stimulation and chronic physiological frequency electrical stimulation on Ca^2+-ATPase activity and Ca^2+ uptale-release kinetics changes of diaphragmatic muscle sarcoplasmic reticulum of "emphysema rabbit. Methods The rabbit model of emphysema was made by inhaling papain of ultrasonic atomization once a week for 3 weeks in 24 adult Japanese rabbits,6 receiving 10Hz chronic low frequency electrical stimulation,6 receiving 10+40Hz, Another 6 emphysema rabbit and 6 rabbits served as control. An enzyme chemical method was used to observe the changes in the enzyme activities of sarcoplasmic reticulum Ca^2+ -ATPase and Fura-2 fluorophotometry was used to assayed the changes in Ca^2+ up- take-release kinetics in control group, emphysema group, emphysema + chronic low frequency electrical stimulation groups. Results Following chronic electric stimulation using 10+40Hz,diaphragmatic muscle SR Ca^2+-ATPase activity was increased in rabbits with emphysema(P〈0.01) ,and Ca^2+ release and uptake rate of SR was significantly increased(P〈0.01). In the 10Hz group, Ca^2+-ATPase activity was lowered,and Ca^2+ release and uptake rate of SR was significantly decreased (P〈0.01). Conclusion Different frequency electrical stimulation induced different adaptative changes in Ca^2+-ATPase activity and Ca^2+ uptale-release kinetics. It indicates that chronic electric stimulation using low and physiologic frequencies may enhance SR Ca^2+-ATPase activity of emphysema rabbit diaphragmatic muscle and increase Ca^2+ release and uptake rate of SR so that it may be one of the optimal frequency mode for the rehabilitation care of diaphragmatic muscle of COPD patients with extraorgan diaphragm pacing.
