改良二孔法腹腔镜阑尾切除术

摘　　要：目的探讨改良二孔法腹腔镜阑尾切除术（LA）的可行性。方法回顾性分析2004年5月至2007年5月开展的改良二孔法LA63例患者（改良二孔组）的临床资料，并与同期开展的三孔法LA（三孔组）62例进行比较。结果改良二孔组63例患者手术全部成功，无一例中转三孔法或开腹阑尾切除术。腹壁上只有两个戳卡孔（10mm或5mm）和一个针孔（1．8mm）。手术操作时间：改良二孔组（48．16±19．34）min，三孔组（47．36±18．63）min；术后自行下床活动时间：改良两孔组（17．56±10．56）h，三孔组（17．67±11．09）h；住院时间：改良两孔组（4．19±1．42）d，三孔组（4．32±1．36）d；术后镇痛药使用：改良两孔组7例，三孔组8例。两组比较差异均无统计学意义（P〉0．05）。结论改良两孔法LA对某些类型的阑尾炎的治疗是一种较好的选择。Objective To explore the feasibility of modified double-trocar hole method of laparoscopic appendectomy （LA）. Method From May 2004 to May 2007,63 cases of modified double-trocar hole LA （modified double-trocar hole group） were analyzed retrospectively,and compared with 62 cases of three-trocar hole LA （three-trocar hole group） of the same period. Results The operations were all completed smoothly, and no one turned to three-trocar hole LA and open appendectomy in modified double-trocar hole group. The operating time was （48.16±19.34） minutes in modified double-trocar hole group and （47.36±18.63 ） minutes in three-trocar hole group, the time of staying in bed was （ 17.56±10.56） hours in modified double-trocar hole group and （ 17.67± 11.09） hours in three-trocar hole group,the hospital length of stay was （4.19 ±1.42） days in modified double-trocar hole group and （4.32 ± 1.36） days in three-trocar hole group, the usage of obtundent postoperatively was 7 cases in modified double-trocar hole group and 8 cases in three-trocar hole group,there were no significant difference between two groups（P 〉 0.05 ）. Abdominal scar：the three-trocar hole LA method had one moi＇e scar than the modified double-trocar hole LA method. Conclusion The modified double-trecar hole LA method is a better option for certain types of appendicitis.
