疏血通注射液对冠心病患者介入治疗前后D-二聚体变化的作用

摘　　要：目的探讨冠心病患者介入治疗前后血浆D-二聚体含量的变化，评价疏血通注射液的干预作用。方法将接受冠状动脉介入治疗术（PCI）的冠心病患者81例随机分成两组，对照组（n=30）给予介入治疗常规用药；治疗组（n=51）在介入治疗常规用药基础上，从术前3天开始至术后第7天，静脉点滴疏血通注射液6ml／d。分别于术前（T0）、术后即刻（T1）、术后第2天（T2）、第5天（T3）、第7天（L）5个时点采血测定D-二聚体（DD），比较两组不同时点的变化及组间差异，并作程序变量与DD变化的相关性分析。结果治疗组于T1、T2时较R时有明显升高（P〈0．05）；对照组于T1、T2、T3时较R时明显升高（P〈0．05），T2时达峰值在高限值2倍左右（P〈0．01），T3时有所下降，但仍明显高于术前（P〈0．05）。两组比较T1、T2、T3时对照组较治疗组有明显差异（P〈0．05）。结论血浆DD的含量升高可能对PCI术后继发性血栓形成有预测价值，可指导进行干预性治疗，静滴疏血通注射液有预防血栓形成作用。Objective To observe plasma levels of D-dimer （DD） before and after percutaneous coronary intervention （PCI） of patients and evaluate the effect of pretreatment with Shuxuetong injection. Methods Eighty-one patients with coronary heart disease undergoing PCI were randomized to control group （n： 30） without Shuxuetong injection before PCI and pretreatment group （n： 51） with Shuxuetong injection （6 ml/d） for three days prior and seven days followed. DD level in plasma was measured at different points： before PCI （T0）, immediately after the PCI （T1）, at two days after PCI （T2）, at five days after PCI （T3） and at seven days after PCI （T4）. The changes within groups at different time points and the differences between groups at same time points were compared. Results There was remarkable elevation in the pretreatment group for the DD plasma level at T1 and T2 （P〈0.05） comparing to T0. The DD plasma level in the control group increased apparently at T1, T2 a n d T3 comparing to TO, reaching peak at T2 （P〈0.01） . T3 still elevated significantly comparing to TO, which were lower than T2 （P〈0.05） .There were remarkable differences in the levels of plasma DD comparing two groups at T1, T2 a n d T3 （P〈 0.05 ）. Conclusions It suggests that the elevation of the DD plasma level may be introduced by PCI and enhance the thrombosis. Pretreatment with Shuxuetong injection can prevent it.
