门静脉栓子实时超声造影研究

摘　　要：目的探讨门静脉栓子的超声造影灌注特点;结合QontraXt定量分析软件,检测门静脉栓子的血流动力学特点。方法应用CnTI技术和造影剂SonoVue对18例门静脉栓子患者进行超声造影检查,动态观察注射造影剂后门静脉栓子的灌注特点;并用QontraXt软件对18例门静脉栓子进行定量分析,分析评价以上各指标在门静脉栓子中鉴别诊断的意义。结果14例门静脉癌栓中,10例呈整体均匀性增强,2例呈低增强,2例呈部分高增强;4例门静脉血栓无1例增强。结论实时超声造影可以敏感地显示门静脉内栓子的动态增强过程,揭示门静脉栓子的血流动力学特点。Objective To investigate the enhanced characteristics of portal vein thrombosis with real-time contrast-enhanced ultrasonography and to explore the hemodynamic characteristics of portal vein thrombosis through qualitative analysis using QontraXt software, Methods With bolus SonoVue injection via peripheral vein, eighteen patients underwent contrast-enhanced ultrasound using real-time gray-scale contrast tuned imaging technology （CnTI） and 14 portal vein tumor thrombosis and 4 portal vein benign thrombosis were found, The enhancement process was assessed. Quantitative analysis on portal vein thrombosis in 18 cases using QontraXt software creating time-intensity curve in which these parameters were analyzed in order to illustrate the significance of real-time contrasted-enhanced ultrasonogrphy in portal vein thrombosis diagnosis. Results All 14 portal vein tumor thrombosis had variously enhanced patterns A homogeneous enhanced pattern was detected in 10 cases wtih portal vein tumor thrombosis A hypoechoic enhanced pattern was detected in 2 cases with portal vein thrombosis A heterogeneous enhanced pattern was detected in 2 cases with portal vein tumor thrombosis. All 4 cases with portal vein benign thrombosis were not enhanced. Conclusions The real time contrast-enhanced ultraonography will be able to sensitively demonstrate the dynamic enhancement of portal vein thrombosis, and to reveal the hemodynamic characteristics of portal vein thrombosis noninvasively.
