Low Dose of Ketamine Treatment Inhibiting Up-regulation of Spinal Calreticulin and IP3 Receptor mRNA in Rats with Spared Nerve Injury

摘　　要：目的观察脊髓钙网蛋白（CRT）和三磷酸肌醇（IP3）I型受体在神经病理性疼痛中转录水平的变化，及有效的小剂量氯胺酮治疗对其影响，探讨它们在神经病理性疼痛中的作用。方法成年雄性sD大鼠18只，随机分成假手术对照组、模型组（SNI组）和治疗组（SNI＋T组），分别接受假手术和保留性坐骨神经分支损伤手术。术后SNI＋T组大鼠每日腹腔注射氯胺酮10mg·kg^-1，对照组和SNI组大鼠每日腹腔注射等容量0．9％氯化钠溶液，并每日检测各组大鼠机械缩爪阈值的变化。术后14d处死大鼠，取脊髓L4-6段，用RT—PCR技术检测CRT、IP3R型mRNA表达的变化。结果与对照组相比，SNI模型组大鼠50％机械缩爪阈值显著降低（P〈0．01）；SNI组大鼠脊髓CRT、IBRI型mRNA表达均上调，与对照组比较差异有显著性或极显著性（P〈0．05或P〈0．01）；氯胺酮治疗组上述变化部分降低（P〈0．05或P〈0．05）。结论大鼠脊髓细胞内钙网蛋白和IP3RI受体上调可能参与了慢性神经病理性疼痛的病理过程。Objective To study the change of transcriptive levels of calreticulin and type I inositol triphosphate receptor （ IP3 R Ⅰ ） in spinal cord of chronic neuropathic pain model, and the effect of ketamine treatment on them. Methods 18 male SD rats were divided randomly into 3 groups （n = 6 ） ： control group received shame operation, model group （SNI） and SNI ＋ T received spared nerve injury （SNI） surgery. 10 mg·kg^-1 ketamine was administrated intraperitoneally to rats in group SNI ＋ T daily, and saline in the equivalent volume was given to rats in other groups. The behavioral change of pain was observed everyday. 14 d after operation, L4-6 of spinal cords was taken out and the mRNA expression of CRT,IP3R I was detected. Results Compared with the control group, 50% paw withdrawal threshold was decreased significantly in the SNI group （t = 18.2, P 〈 0.01 ）. The up-regulation of calreticulin mRNA and IP3R I mRNA was found in the SNI group （t = 4.78, P 〈 0. 05 and t = 4.46,P 〈 0.05 ）, compared with the control group. And the changes were partially inhibited by the ketamine treatment. Conclusion Up-regulation of calreticulin mRNA and IP3R I mRNA might play a role in the development of chronic neuropathic pain induced by SNI.
