桂乌汤对类风湿性关节炎的内分泌免疫调节作用

摘　　要：目的:从调节内分泌免疫网络着手探讨桂乌汤治疗类风湿性关节炎(RA)的作用机制。方法:RA患者共83例,随机分为三组。中药组服桂乌汤,中西结合组服中药和甲氨喋呤(MTX),西药组单用MTX。3个月后取血测RF、ESR、CRP、IgG、IgA、IgM、补体C3、T淋巴细胞亚群、E2、CS水平。结果:中西医结合组RF、CRP水平明显低于中、西药组(P<0.05);各组治疗后CD8+水平明显下降(P<0.05),CD4+/CD8+明显升高(P<0.05);中西医结合组CD8+水平、CD4+/CD8+值与中、西药组间比较差异均有显著性意义(P<0.05):中药组治疗后E2、CS水平明显高于中西医结合组和西药组(P<0.05)。结论:桂乌汤能有效调节E2、CS等内分泌激素水平,影响CD4+/CD8+比值及IgA、IgM等免疫分子水平,降低RF、ESR、CPR水平,从而有效控制炎症和缓解关节症状。Objective：To study the incretion-immune effect of Guiwu decoction on Rheumatoid Arthritis. Methods： Patients in Chinese medicine group took orally Guiwu decoction. Patients in integrated traditional and western medicine group took orally Guiwu decoction and MTX. Patients in western medicine group took orally MTX only. 3 month later, the levels of RF, ESR, CRP, IgG, IgA, IgM and C3 in serum were detected respectively, the level of sub-T lymphocyte was detected with flow cytometry and the levels of E2 and CS were determinated by radio immunoassay. Results：The levels of RF and CRP were lower than those in Chinese medicine group and western medicine group （ p 〈 0. 05 ）. The level of CD8 ＋ rose while the level of CI）4 ＋/CD8 ＋ decreased obviously after treatment （ P 〈 0.05 ）. The levels of CD8 ＋ and CI）4 ＋/CD8 ＋ in integrated traditional and western medicine group were obviously different from those in Chinese medicine group and western medicine group（ P 〈 0.05 ）. The levels of E2 and CS in Chinese medicine group were obviously higher than those in integrated traditional and western medicine group and western medicine group （P 〈 0.05 ）. Conclusion： Guiwu decoction can effectively adjust the level of hormone such as E2 and CS, influence the levels of CD4 ＋/CD8 ＋ , IgA and IgM and decrease the levels of RF, ESR and CRP, then effectively control the inflammation and relieve the syndrome of arthritis.
