益气活血养阴方在老年肺癌放疗中的作用

摘　　要：目的探讨益气活血养阴方在老年肺癌三维适形放射治疗中的作用。方法将94例三维适形放疗V30≥20的肺癌患者随机分为治疗组与对照组,剂量-分次-时间:2.0～2.5Gy/次,5次/周。放射总剂量与时间:非小细胞肺癌为60～66Gy,6～8周完成治疗;小细胞肺癌为46～50Gy,4～5周完成治疗。治疗组服用益气活血养阴方,每日1剂,放疗开始至放疗结束后8周,对照组服用氨溴索,时间相同。结果治疗组与对照组肿瘤CR率分别为63.8%和42.1%,治疗组与对照组放射性肺损伤发生例数分别为8例和14例,持续时间分别为(17.18±5.8)d和(25.83±5.6)d,体积分别为(618±114)cm3和(827±132)cm3,2～3级放射性食管炎发生率分别为40.4%和63.2%,差异有显著性(P均<0.05)。放射治疗前后治疗组与对照组生活质量、Karnofsky评分、肺损伤的分子生物学指标相比较,差异均有显著性(P均<0.05)。讨论益气活血养阴方能预防和治疗放射性肺损伤,减少食管炎的发生,提高肺癌的治疗效果,改善患者生活质量,延长生存期。Objective To study the effect of Yiqi Huoxue Yangyin Recipe（YHYR） on elderly man with lung cancer receiving three-dimensional eonformal radiotherapy （3DCR）. Methods Ninety-four patients with lung cancer,who received 3DCR V30 〉 20, were divided randomly into two groups ：treated （T） group and control（C） group. Dose-fraction-time ：2.0-2.5 Gy/fraction and five fractions every week. For nonsmall cell lung cancer （NSCLC）, the total irradiation dose was 60-66Gy and the treatment was completed in 6-8weeks,while for SCLC, the total irradiation dose was 46-50Gy and the treatment was completed in 4-5 weeks. T group took YHYR, one dose everyday, from before radiotherapy to 8 weeks after radiotherapy, while C group took ambroxol hydroehloride at the same time. Results The CR rate was 63.8% in T group and 42.1% in C group. The number of cases of radiation-induced lung injury （RILI） was eight and fourteen in T group and C group respectively, the duration was 17.18 ± 5.8 days and 25.83 ± 5.6 days and the volume was 618 ± 114 cm^3 and 827 ± 132 cm^3. The incidence of radiation-induced esophagitis of grade 2 and 3 in T group and C group was 40.4% and 63.2% respectively, with significant difference （P 〈 0.05 ）. In addition, the life quality, KPS, and the molecular biology index of RILI had significant difference （ P 〈 0.05 ） in T group and C group before and after radiotherapy. Conclusion YHYR can prevent and treat RILl, reduce the occurrence of esophagitis,enhance the curative effect of lung cancer, improve the life quality of patients and prolong survival time.
