特重型颅脑损伤84例救治探讨

摘　　要：目的探讨特重型颅脑损伤救治的方法与效果。方法对我科自1996年8月～2005年7月救治的84例特重型颅脑伤患者临床资料进行回顾性分析。结果本组存活55例(65.48%),随访并经ADL统计,恢复良好30例(54.55%),中、重残21例(38.18%),植物生存4例(7.27%)。结论尽早明确诊断,及时开颅清除血肿、去大骨瓣充分减压是救治特重型颅脑伤的关键;颞肌敷贴、早期使用亚低温及钙离子阻滞剂等对脑的保护、损伤可逆脑细胞的修复和脑功能的恢复有良好的促进作用;脱水,止血,抗感染、营养脑神经、维持水电解质及酸碱平衡等亦是提高特重型颅脑伤患者的生存率的保证。Objective The study saves he method that cure with the heavy type skull brain wound and result. Methods The saves to cure from 1996 August to 2005 July of a my section of a with special heavy type skull brain wounded person the clinical data enters the sex review analysis. Results This set survives 55 examples （65.48%）, with vist to combine to was statisticsed by ADL, Recover good 30 examples （54.55%）, inside 21 examples：（38.18%） midding heavy maimed, the plant exists 4 examples （7.27%）. Conclusions As early as possible clear and definite diagnosis, open the skull clearance blood in time swollen, go to the big bone petal well the decompression is a key to save to cure to harm with the special heavy type skull brain. The temporal muscle application sticks, second low temperature in usage in earlier period and calcium ion block an etc. To the protection, harm of the brain can the inverse brain cell repair to have to promote the function goodly with the instauration of the brain function; dyhydrate,stop bleeding,anti-infection, nourishment brain nerve,maintain water electrolyte and equilibrium etc. In sour alkali as well is increase with heavy type skull brain sick and wounded person of exist the rate guarantees.
