摘 要:目的:分析缺血性卒中TOAST分型各亚型DWI显示的病灶特点。方法:对我院住院经磁共振成像诊断的缺血性卒中患者共262例,按照TOAST分型进行病因学分类,分析各亚型患者DWI所显示病灶的方式及特征。结果:缺血性卒中TOAST分型中,各型依次为SAA28.62%,LAA28.24%,SUE15.64%,SOU15.26%,CE12.21%;DWI未显示病灶20例(7.61%),SAA占55%,显示单个病灶130例(49.61%),≥1.5cm者LAA占46.55%,<1.5cm者SAA占71.62%,散发病灶者67例(25.57%),除CE较少外,各型之间无显著差异,多发病灶45例(17.17%),其中CE和LAA均为18.96%。结论:基于DWI显示的病灶特点,有助于缺血性卒中病因的阐明。Objective:To analysis lesion patterns on DWI in isehemie stroke TOAST subtypes. Methods:262 patients with isehemie stroke, diagnosed by MRI were classfied retrospectively according to TOAST subtype ciriteria, we analysed the characteristics and lesion pattens in different aetiologies of ischemic stroke. Results : The most common underlying stroke subtype, in 75 (28.62%) patients was SAA ,LAA in 74(28.24% ) patients, CE in 32( 12.21% ) patients,SOU in 40( 15.26% ) patients,SUE in 41 ( 15.64% ) patients. According to lesion patterns on DWI, No lesions on DWI were found in 20 (7.61%)patients, mainly in SAA (55%). Single lesion in 130(49.61% ) patients were divided into ≥ 1.5cm in LAA (46.55%), and 〈 1.5cm in SAA (71.62%). Scatted lesions in 69 (26.33%) patients were not speeific to any one elassficationg TOAST Subtypes, beside CE. Multiple lesions in 45 ( 17.17% ) patients were in CE and LAA ( 18.96% , respectively). Conclusions:Lesion patterns based on DWI were helpful for defining aetiology of isehemic stroke.
