摘 要:目的:探讨尿毒症合并顽固性腹水低温下超滤浓缩回输腹腔对患者免疫功能的影响。方法:纳入41例尿毒症合并顽固性腹水住院患者,随机分为低温组(36℃)21例和常温组(37℃)20例进行腹水超滤浓缩回输治疗,并检测治疗当日和次日晨空腹血T淋巴细胞亚群及白细胞介素-2水平。结果:尿毒症合并顽固性腹水患者常温、低温腹水超滤浓缩回输腹腔治疗后CD4+、CD4+/CD8+、IL-2水平均明显高于治疗前(P<0.01,P<0.05,P<0.01),但常温、低温组间比较CD4+、CD4+/CD8+、IL-2水平差异无显著性。结论:腹水超滤浓缩回输腹腔能明显改善尿毒症合并顽固性腹水患者机体免疫功能,回输腹水的温度对患者体液和细胞免疫变化未产生明显的影响。Objective:To discuss the effect of low-temperature ascites ultrafiltration-concentration-reinfusion method on the immunologic functions of uremic patients with refractory ascites.Methods:A total of 41 cases of hospitalized uremic patients with refractory ascites were divided into two groups randomly:the low-temperature(36℃) group of 21 cases and the normal-temperature(37℃) group of 20 cases.The patients of both groups were given their respective treatment by low-temperature or normal-temperature ascites ultrafiltrationconcentration-reinfusion method.On the morning of the same day and the next day of treatment, the patients all had their blood tests on an empty stomach for the examination of the T lymphocyte subpopulation and interleukin-2 level.Results:After the respective treatment by low-temperature or normal-temperature ascites ultrafiltration-concentration-reinfusion method,the levels of CD4^+,CD4^+/CD8^+ and IL-2 (P〈 0.01 ,P〈0.05 ,P〈0.01) of all the uremic patients with refractory ascites were all obviously higher than those before the treatment.However, in contrast there was no remarkable difference in the levels of CD4^+, CD4^+/CD8^+ and IL-2 between the low-temperature and normal-tem- perature groups.Conclusion:The ascites ultrafiltration-concentration-reinfusion method can obviously improve the immunologic functions of uremic patients with refractory ascites.However,the temperature of the ascites for traperitioneal reinfusion has no obvious effect on the changes of the patient's humoral immunity and cytoimmunity.
