副鼻窦炎的CT诊断价值

摘　　要：目的:探讨CT在副鼻窦疾病临床诊断中的价值。方法:对186例经CT横断扫描和(或)冠状扫描,同时做了X线平片检查的副鼻窦病例进行回顾性分析。结果:全组病例以鼻阻、头痛、脓涕或回缩性涕血为主要症状。CT显示病变的鼻窦腔内窦壁黏膜增厚、窦腔积液、黏膜下囊肿、黏液囊肿、息肉等,其中单纯黏膜增厚45例,窦腔积液23例,黏膜下囊肿6例,黏液囊肿3例,复合型病变(包括以上两种或两种以上改变)87例,阴性22例;X线平片显示病变的窦腔黏膜增厚、窦腔积液、囊肿样改变、窦腔普遍密度增高等,其中黏膜增厚34例,窦腔积液21例,囊肿样改变10例,窦腔普遍密度增高81例,阴性40例。结论:CT检查较X线平片检查更能清晰显示鼻窦病变的影像表现特征,特别是避免了前后或左右解剖结构的重叠,能清晰显示副鼻窦的解剖及变异,以及周围的毗邻关系,为临床治疗提供了良好的诊断依据。Objective：To analyse the clinical manifestations of CT and X-ray plain film in paranasal sinusitis and investigate the value of CT in the clinical diagnosis of paranasal sinusitis.Methods： 186 patients with paranasal sinusitis undergoing the CT examination of transverse scan and/or coronary scan as well as the examination of X-ray plain film at the same time were analysed retrospectively.Resuits：The main symptom in this group was nasal obstruction,headache,purulent nasal discharge and rebound bleeding.CT showed the mucosal thickening, submucous cyst, mucous cyst, hydrops, polyp etc.in the paranasal sinus cavity,in which simple mucosal thickening was 45 cases, sinus cavity hydrops 23 cases, submucous cyst 6 cases,mucous cyst 3 cases, compound pathological change（including two or over two kinds of the foregoing change） 87 cases and negative 22 cases;X-ray plain film showed the mucosal thickening,cystic change,hydrops,universal density raising shadow,etc.in the paranasal sinus cavity,in which simple mucosal thickening was 34 cases,sinus cavity hydrops 21 cases,cystic change 10 cases,universal density raising 81 cases and negative 40 cases.Conclusion：Compared with the examination of X-ray plain film,CT could display the image characteristics of paranasal sinus pathological change more clear,especially in avoiding the duplication of anterior-posterior and left-right anatomical structure.It could clearly display the dissection and variation of paranasal sinus as well as its peripheral adjacent relations,and provide the good diagnostic evidence for the clinical treatment.
