摘  要:目的:研究血清血管内皮生长因子(VEGF)水平在冠心病(CHD)诊断和冠状动脉介入治疗(PCI)中的临床意义。方法:将CHD组62例和非CHD组(对照组)20例,患者行冠状动脉造影(CAG)。CHD组中,按照美国心脏病学会和美国心脏协会(ACC/AHA)2000年解剖风险分组法分为低危(A亚组,23例),中危(B亚组,21例),高危(C亚组,18例)3组,组中部分患者行即刻PCI。使用酶联免疫吸附测定法(ELISA)检测所有患者CAG前和术后(包括CAG或PCI)0.5h血清VEGF浓度。结果:在CHD组中血清VEGF水平明显高于对照组(P<0.01),A亚组与对照组差异无统计学意义(P>0.05),但明显低于B组(P<0.05)和C组(P<0.01)。B组,C组均明显高于对照组(P<0.01),C亚组明显高于B组(P<0.01)。单纯行CAG未作PCI者(60例,包括对照组)前后VEGF血清浓度差异无统计学意义(P>0.05),CAG后即行PCI组(22例)术后明显低于术前(P<0.01),且术后VEGF水平与对照组差异无统计学意义(P>0.05)。结论:CHD患者血清VEGF浓度明显高于非CHD患者,血清VEGF浓度与CHD患者冠状动脉病变的危险分层有密切关系且PCI后明显降低。Objective:To investigate the clinical significance of level of serum vascular endothelial growth factor (VEGF) in PCI and diagnosing CHD. Method:A total of 82 patients (62 patients in the CHD group with significant coronary artery disease; 20 patients in the D group without coronary artery disease) were enrolled and referred for coronary angiography(CAG). In the CHD group, according to the coronary angiography and interventions lesion classification system AHA/ACC at 2000, the patients were classified the low degree of risk(Group A, n=23), the middle degree of risk(Group B,n=21) and the high degree of risk(Group C,n=20). Serum samples were obtained before CAG and after CAG or PCI. VEGF were measured by enzyme-linked immunosorbent assay (ELISA). Result: The concentration of serum VEGF in the CHD group was higher(P〈0.01) than that in the non- CAD group. The concentration of serum VEGF was similar between A group and non-CAD group(P〉0.05). It was lower in A group than in B group(P〈0.05) or in C group(P〈0.01). It was higher in B or C group than in non-CHD group(P〈0.01) and it was significantly higher in C group than in B group(P〈0.01). It was lower in the group after PCI in all patients with PCI than in the group before CAG(P〈0.01). The concentration of serum VEGF was similar between before and after CAG in all patients without PCI(P〉0.05); It was similar between the group after PCI in all patients with PCI and the non-CAD group (P〉0.05). Conclusion: The concentration of serum VEGF in the CHD group was higher than in the non-CHD group. The concentration of serum VEGF correlated with the degree of risk of PCI in CHD patients, and decreased immediately after PCI.
