卵巢癌中耐药标志的表达及临床意义

摘　　要：目的研究P-糖蛋白(p-gp)和谷胱甘肽S转移酶π(GST-π)与肺耐药相关蛋白(LRP)在卵巢上皮性癌中的表达及其对化疗疗效的影响。方法选择中国医科大学附属第一医院1998年1月至2004年5月存档蜡块,应用免疫组化方法对70例卵巢上皮性癌进行p-gp、GST-π、LRP检测。结果(1)p-gp、GST-π、LRP卵巢上皮性癌中的阳性表达分别为31.4%(22/70)、65.7%(46/70)、58.6(41/70),与其在正常卵巢组织、卵巢良性肿瘤中的表达差异均有统计学意义(P<0.05)。(2)p-gp、GST-π、LRP的表达与临床分期、组织学类型均无相关性。(3)对首次术后有残余病变的42例患者进行化疗评价,p-gp阳性组与阴性组对化疗的有效率为40%及66.7%,二组比较差异无统计学意义(P>0.05);GST-π阳性组与阴性组对化疗的有效率为42.3%及81.3%,差异有统计学意义(P<0.05);LRP阳性组与阴性组对化疗的反应率为40%及82.4%,差异有统计学意义(P<0.05)。(4)p-gp、GST-π、LRP存在共表达。结论GST-π、LRP阳性表达者化疗疗效差,检测GST-π、LRP对前瞻性预测化疗耐药有一定价值。Objective To study the expression of P-glycoprotein（p-gp） ,glutathione S-transferase-π（GST-π） and Lung resistant-related protein （LRP）in epithelial ovarian cancer;in addition,assess their predictive value as indicators of resistance to chemotherapy. Methods The expression of p-gp,GST-π and LRP in paraffin sections of 70 cases of epithelial ovarian cancer was studied using immunohistochemical staining. Results （ 1 ） The positive expression of p-gp , GST-π and LRP in epithelial ovarian cancer was 31.4% （ 22/70 ） ,65.7% （ 46/70 ） ,58.6 （ 41/70 ）, respectively. The difference between them and their expressions in normal ovaries and ovarian benign tumor was statistically significant （ P 〈 0.05 ）. （2） The expession of p-gp,GST-π and LRP was not associated with clinical stage and histologic type. （3）For 42 postoperative residual ovarian cancer patients ,the response rate to chemotherapy of the p-gp positive group and negative group was 40% and 66.7% ,the difference being not statistically significant. （P 〉 0.05 ）. The response rate to chemotherapy of the GST-π positive group and negative group was 42.3% and 81.3% ,the difference being statistically significant, （P 〈0. 05）. The response rate to chemotherapy of the LRP positive group and negative group was 40% and 82.4% ,the difference being statistically significant （P 〈 0.05 ）. （4） p-gp ,GST-π and LRP may express in one case. Condusion The patients with GST-π and LRP postively stained show unsatisfied result to chemotherapy ,and detection of CST-π and LRP is useful in assessing resistance to chemotherapy.
