摘 要:目的:了解成都地区铜绿假单胞菌的耐药率及其分布与流行情况,指导临床用药。方法:采用法国生物梅里埃公司VITEK-32全自动细菌鉴定仪或常规方法进行细菌鉴定,VITEK-32药敏分析系统革兰阴性菌鉴定卡测定铜绿假单胞菌对14种抗菌药物的敏感性,部分菌株采用Kirby-Bauer法进行药敏试验。结果:铜绿假单胞菌呼吸道感染最为常见,占63.90%,其次为伤口感染(18.05%)。对头孢呋辛、氨苄西林/舒巴坦的耐药率均超过80%。对复方新诺明、头孢他啶、哌拉西林、头孢吡肟、庆大霉素和亚胺培南的耐药率也分别高达78.54%、44.88%、46.83%、44.88%、48.29%和36.59%。最敏感的药物是阿米卡星(敏感率为88.78%)。205株铜绿假单胞菌中有75株菌(36.59%)对14种抗生素中的10种以上耐药。结论:成都地区临床分离的铜绿假单胞菌耐药及多重耐药现象严重,临床医师应根据药物敏感试验和本地区耐药监测情况,合理选择抗菌药物。Objectives: To assess the distribution and prevalence of drug-resistance rates among Pseudomonas aeruginosa isolates in Chengdu. Methods: Strains of P. aeruginosa were identified by VITEK-32 automatic microanalyzer and by conventional biochemical tests. The resistance to 14 antibiotics of P. aemginosa was determined by a microdilufion broth method. Drug sensitivity testing (Kirby- Bauer) was also conducted as supplementary for some strains. Results: The two foremost infection sites were respiratory tract 163.90% ) and wound ( 18.05% ) . More than 80% of the isolates were resistant to cefuroxime, and mpicillin/sulbactam. The resistant rates to sulphamethoxazole/trimethoprim, ceftazidime, piperacillin, cefepime, gentamicin and imipenam were 78.54%, 44.88%, 46.83%, 44.88%, 48.29% and 36.59%, respectively. AMK was the most sensitive to P. aernginosa (the sensitivity rate of P. aernginosa for AMK was 88.78% ) . Among 205 strains of P. aeruginosa, 75 (36.59% ) were resistant to over 10 of 14 antibiotics. All of the 205 isolates were resistant to over 3 antibiotics. Conclusions: Tne resistance and multiple drug resistance of Pseudomonas aeruginosa isolated from clinical samples in Chengdu is a serious issue. Clinicians should select appropriate antibiotic therapy based on sensitivity testing . The therapeutic strategy should he adapted according to the local ecology of resistance.
