老年睡眠呼吸暂停低通气综合征患者生活质量的相关性研究

摘　　要：目的调查阻塞性睡眠呼吸暂停低通气综合征（OSAHS）对老年人生命质量的影响及影响因素。方法利用一般健康状况调查问卷（SF-36）健康评价量表评估生活质量,病历对照研究比较分析OSAHS患者和对照组在SF-36各健康领域功能状态的差异,线性相关分析多导睡眠监测（PSG）相关参数和SF-36各领域分值的相关性和相关程度。结果中重度OSAHS患者SF-36中躯体功能、生命力及社会功能领域分值明显低于对照组（P〈0.05）。相关分析发现,躯体功能和年龄、整夜脉搏血氧饱和度〈90%时间呈负相关,生命力与整夜脉搏血氧饱和度〈90%时间、Epworth嗜睡评价量表（ESS）呈负相关,社会功能与ESS呈负相关（P〈0.05）。结论中重度OSAHS对老年人的生命质量有负面作用,年龄、夜间低氧血症持续时间和白天嗜睡可能起了较大的作用。Objective To investigate the effect of obstructive sleep apnea hypopnea syndrome （OSAHS）on the life quality of the elderly and the influencing factors.Methods The quality of life was assessed by MOS 36-item short form health survey（SF-36）,and the comparison between the OSAHS group and the control group was made,polysomnogram（PSG）parameters and the SF-36 in OSAHS patient were analyzed by linear correlation analysis.Results Physical function,vitality and social function of the SF-36 in severe OSAHS group were obviously lower than in the control group （all P〈0.05）.Correlation analysis showed that physical function was inversely correlated with age and time of total sleep spent at SpO2〈90%,vitality was inversely correlated with the time of total sleep spent at SpO2〈90% and the scale of excessive daytime sleepiness（ESS）,social function was inversely correlated with epworth sleepiness scale（ESS）（all P〈0.05 ）.Conclusions Severe OSAHS plays the negative role in elderly persons＇ life quality.The age,time of total sleep spent at SpO2〈90% and ESS may play a major part in the abnormalities.
