CT体检对腹部器官疾病检出情况分析

摘　　要：目的探讨腹部器官CT体检的疾病检出率和不同年龄组疾病检出情况。方法以493例年龄42～102岁,无临床症状的CT体检者为研究对象,统计腹部各脏器疾病的检出率,同时对不同年龄组的患病情况进行比较。CT扫描全部为平扫,扫描范围从右侧膈肌顶部开始至双肾下极,CT平扫采用5 mm层厚、5 mm间隔连续扫描。结果493例中,实性脏器疾病检出率前5位为肾囊肿（49.7%）、肝囊肿（49.3%）、脂肪肝（19.1%）、胆结石（15.2%）和肾结石（9.5%）。经趋势χ2检验,脂肪肝、肝血管瘤、胆囊结石、肾囊肿、肾结石、肾上腺腺瘤随受检者年龄增长患病检出率增加,患病检出率与年龄呈正相关（均为P〈0.05或P〈0.01）。空腔脏器以食道裂孔疝（3.9%）和十二指肠憩室（3.2%）居多。结论腹部器官CT体检疾病的检出率较高,常见疾病为肾囊肿、肝囊肿、脂肪肝、胆结石、肾结石及食道裂孔疝、十二指肠憩室。脂肪肝、肝血管瘤、胆囊结石、肾囊肿、肾结石、肾上腺腺瘤的患病检出率与年龄呈正相关。Objective To investigate the detection rate of diseases with abdominal CT for routine health examination and analyze the detection situation of different age groups.Methods There were 493 persons who had no clinical symptoms and accepted CT for routine health examination.The detection rates of diseases of different abdominal organs were statistically analyzed and the situations of diseases in different age groups were compared.Plain or enhanced-CT scan was used,scanning from the top of the right diaphragm to the inferior pole of kidney.Continuous scan of 5-mm thickness and 5-mm interspace was used on plain scan.Enhanced CT scan used 2 phases with 10-mm thickness and 10-mm interspace on arterial phase and 5-mm thickness and 5-mm interspace on venous phase.Results Among the 493 cases by trend χ2 test,the top 5 diseases detected in parenchymal organs were renal cysts（49.7%）,liver cysts（49.3%）,fatty liver（19.1%）,gallstone （15.2%）and renal lithiasis（9.5%）.The morbidity of fatty liver,hepatohemangioma,gallstone,renal cyst,renal lithiasis and adrenal adenoma were increased with aging and the morbidity had positive correlation with age（respectively,all P〈0.05 or P〈0.01）.The diseases of luminal organs were mainly esophageal hiatus hernia（3.9%）and duodenal diverticulum（3.2%）.Conclusions The detection rate of diseases on routine abdominal CT is high.The common diseases are renal cyst,liver cyst,fatty liver,gallstone,renal lithiasis,esophageal hiatus hernia and duodenal diverticulum.The morbidity of fatty liver,hepatic hemangioma,gallstone,renal cyst,renal lithiasis and adrenal adenoma has positive correlation with ageing.
