非吸烟Ⅱ型糖尿病患者的肺功能异常

摘　　要：测定63例非吸烟Ⅱ型糖尿病患者的各项肺功能指标，分析可能的影响因素。其中有慢性并发症组36例，无慢性并发症组27例，采用放射免疫法测定尿微量白蛋白，并测定各项肺功能指标。结果：Ⅱ型糖尿病患者各项肺功能值均低于相应的预计值；各项肺功能下降与病程呈负相关；肺弥散功能下降与代谢控制水平密切相关。有慢性并发症组各项指标均低于无并发症组;且低氧血症发生率明显高于无并发症组。结论：Ⅱ型糖尿病患者具有肺功能异常，肺功能的主要影响因素是糖尿病的病程和代谢控制水平。To investigate the changes of lung functions in type Ⅱ-diabetes (NIDDM) and the relationship between those functions and diabetes duration and the level of metabolic control, 63 patients with NIDDM were studied in this study.METHODS Hospitalized patients (38 female, 25 male) with age of 20-79yr. were selected. Among them,36 with one of the diabetic chronic complications such as retinopathy,nephropathy or neuropathy were allocated in the complication group, while the other 27 with no signs of any chronic complications were in the control group.RESULTS The results are the followings: the mean values of vital capacity (VC), inspiratory capacity (IC),total lung capacity (TLC), forced vital capacity (FVC), forced expiratory volume in one second (FEVI), and diffusion of lung with carbon monoxide (DLCO) of diabetic patients were lower than the predicted values; the diabetic duration was negative correlated with VC, IC, TLC, FVC and DLCO; The level of HbAlc assayed at the enrollment was negative correlated with DLCO; The values DLCO in the complication group were lower than those in the control group. The incidence of mild hypoxymia (Pao2<11.3 kPa found in the complication group (15 out of 36, 41.67% ) was significantly higher than that in the control group (4 out of 14.81%).CONCLUSIONS Abnormal lung functions exist in patients with NIDDM. Main effective factors to lung functions are the diabetic duration and metabolic control level.
