慢性乙型肝炎患者血清TGF-β、MMP-1、TIMP-1水平及其与肝纤维化程度的研究

摘　　要：目的:检测慢性乙型肝炎患者血清转化生长因子-β(TGF-β)、基质金属蛋白酶-1(MMP-1)、基质金属蛋白酶组织抑制物-1(TIMP-1)水平及变化和肝纤维化程度及其变化,探讨其血清TGF-β、MMP-1、TIMP-1与肝纤维化及其进展的关系。方法:慢性乙型肝炎患者80例,行肝穿刺活检,病理组织进行纤维化程度半定量计分,6个月后复查;检测血清TGF-β、MMP-1、TIMP-1、IV型胶原(ⅣC)、层黏连蛋白(LN)、Ⅲ型前胶原N端肽(PⅢP)、透明质酸酶(HA)6个月后复查。结果:肝纤维化分级不同,血清TGF-β、TIMP-1、TIMP-1/MMP-1差异显著(P<0.01),MMP-1差异不明显(P>0.05);TGF-β(r=0.745,P<0.01)、TIMP-l(r=0.623,P<0.01)、TIMP-1/MMP-l(r=0.336,P<0.01)纤维化分级成正相关,MMP-l与纤维化分级成负相关(r=-0.465,P<0.01),肝纤维化与血清PⅢP、HA显著正相关;61例纤维化程度及计分无显著增加,血清ⅣC、LN、PⅢP、HA无明显变化,血清TGF-β、MMP-1、TIMP-1较低且变化不明显(P>0.05);19例患者炎症活动度及纤维化程度计分增加,TGF-β、TIMP-1、TIMP-1/MMP-1比值较高(P<0.01),且升高显著(P<0.01),MMP-l无显著差别,变化也不明显。结论:血清TGF-β、TIMP-1、MMP-1、TIMP-1/MMP-1水平及动态变化可作评估肝纤维化程度及发展指标。Objeetive ：To investigate the relationship of serum transforming growth factor-β, matrix metaUopreteinases-1 and tissue inhibitors of metallopreteinase-1 with the severity of liver fibresis.nethods：A total of 80 patients with HBV related liver fibrosis and early fibrosis were enrolled from one hospital.Serum fibrosis markers including hyalurenic acid （HA）,Ⅳ collagen （Ⅳ-C）,aminoterminal propeptide of type Ⅲ precollagen（PⅢP）,laminin（LN）,matrix metallopreteinases-1（MMP-1 ）and tissue inhibitors of metallopreteinase-1 （TIMP）-1 and transforming growth factor-β （TGF-β） Levels were determlned.hver fibrosis were assessed according to a modified Scheuer and Chevallier＇sscoring system.Re.suits：In the patients of different stages of liver fibrosis,serum TGF-β ,TIMP-1 and TIMP-1/MMP-1 were different （P〈0.01） and serum MMP-1 was not remarkably different. Serum TGF-β（r=0.745,P〈0.01）,TIMP-1 （r=0.623,P〈0.01） and TIMP-1/MMP-1/（r=0.336,P〈0.01） were correlated positively with the degree of hepatic fibrosis,whereas serum MMP-1（r=-0.465,P〈0.01） was correlated negatively.In The patients without remarkably increasing hepatic fibrosis degree,serum TGF-β,TIMP-1/MMP-1 and TIMP-1 were lower and had not increased.Conclusion：Serum TGF-β,TIMP-1 and TIMP-1/MMP-1 can be used as indices of evaluating hepatic fibrosis degree and growth.
