瑞芬太尼复合异丙酚静脉麻醉在游离皮瓣移植术中的应用

摘　　要：目的评价瑞芬太尼复合异丙酚静脉麻醉在游离皮瓣移植术中的临床效果及安全性。方法选择ASAⅠ～Ⅱ级需行游离皮瓣移植术病例40例,随机分成R组(瑞芬太尼复合异丙酚组)和F组(芬太尼复合异丙酚组)各20例。入手术室后静脉注射咪唑安定0.05mg/kg诱导后,R组:瑞芬太尼0.25μg/(kg·min)、异丙酚100～200μg/(kg·min);F组:芬太尼0.25μg/(kg·min)、异丙酚100～200μg/(kg·min)持续泵入维持麻醉,观察相关指标。结果2组术中麻醉效果差异无显著性(P>0.05),术后复苏均安静,R组镇痛作用强于F组,R组术后恢复明显比F组快(P<0.01)。结论在游离皮瓣移植术中瑞芬太尼比芬太尼更优越、更安全。[Objective] To evaluate the anesthesia effect and saftey of remifentanil-propofol intravenous anesthesia in the operation of free skin flap. [Methods] 40 cases undergoing the operation of free skin flap were randomly divided into R group（remifentanil 0.25 μg/kg· min and propofol） and F group （fentanyl 0.25 μg/kg, rain and propofol） of 20 cases each, The relative data were observed. [Results] There was no significant difference between two groups in anesthesia effect （P 〉0.05）. But the analgesia effect in R group was better than F group and quickly recovered （P 〈0.01）. [Conclusions] The effect and safety of remifentanil might be than fentanyl in the operation of free skin flap.
