一期修复男性尿道下裂167例临床分析

摘　　要：目的总结不同术式一期手术修复男性尿道下裂的疗效。方法男性先天性尿道下裂203例，其中一期手术修复167例。手术方式：MAGPI术3例，Mathieu术8例，尿道延伸术27例，Snodgrass术18例，Onlay术6例，Duckett术26例，游离膀胱黏膜尿道成形5例，游离腹膜代尿道1例，阴囊中缝带蒂皮瓣尿道成形57例，Duplay术＋Duckett术16例。结果一期修复成功率78．44％，并发症发生率21．56％，再手术率11．38％。其中MAGPI术一期修复成功率为100％，Mathieu术75、00％，尿道延伸术74．07％，Snodgrass术77．78％，Onlay术66．67％，Duckett术76．92％，膀胱黏膜代尿道40．00％，腹膜代尿道0，阴囊中缝带蒂皮瓣尿道成形85．96％，Duplay＋Duckett术81．25％。结论根据不同病变程度设计个体化手术方案，尿道下裂一期修复疗效满意。Objective To evaluate the efficacy of various surgical procedures for the one-stage repair of male hypospadias. Methods Among 203 cases of male inborn hypospadias 167 cases were treated by one stage repair. Of them, MAGPI was in 3 cases, Mathieu in 8, Urethra-lengthening technigue in 27, Snodgrass （ Tubularized in- cised plate ） urethroplasty, in 18, Onlay island flap in 6, Duckett procedure in 26,bladder mucosa graft urethroplasty in 5 ,Peritoneum graft urethroplasty in 1, scrotal septal vascular pedicle flap in 57 and Duckett plus Duplay tech- niques in 16 were adopted. Results The operative success rate for all cases was 78.44% ,the morbidity of all complications was 21.56%, and the rate of reoperations was 11.38%. The success rate of MAGPI, Mathieu, Urethra- lengthening technigue, Snodgrass, Onlay, Duckett, bladder mucosa graft urethroplasty, Peritoneum graft urethroplasty, scrotal septal vascular pedicle flap,and Duplay ＋ Duckett was 100% ,75% ,74.07% ,77.78% ,66.67% ,76.92%, 40% ,0,85.96% ,and 81.25% respectively. Conclusion The selection of various surgical methods according to individual patient＇s conditions should be considered. The efficacy of one-stage repair of hypospadias is satisfactory.
