摘 要:目的探讨受体血预先经门静脉输入供体对大鼠肝移植免疫排斥反应的作用。方法采用LEW和ACI大鼠作为肝移植的受体和供体。实验分为4组,Ⅰ组,无处理组;Ⅱ组,移植前7d将受体血1ml经阴茎静脉输入供体;Ⅲ组,移植前7d将非受体大鼠血(BN大鼠)1ml经门静脉输入供体;Ⅳ组,移植前7d将受体血1ml经门静脉输入供体。移植后观察大鼠生存时间;检测血清γ干扰素(IFN-γ)浓度;检测移植肝内树突状细胞数量,进行肝组织学观察;检测移植肝内浸润淋巴细胞的凋亡。结果移植前7d受体血经门静脉输入供体组的生存时间显著延长,为(33.7±2.6)d,无处置组为(10.1±0.7)d。移植前7d受体血经门静脉输入供体组的血清IFN-γ显著升高,移植肝汇管区内的淋巴细胞浸润显著减少,移植肝内检测到多量供体来源的树突状细胞。移植前7d受体血经门静脉输入供体组的肝组织的浸润淋巴细胞凋亡数显著多于无处置组。结论受体血预先经门静脉输入供体,可以延长异系大鼠肝移植生存时间,移植肝内浸润淋巴细胞凋亡可能抑制排斥反应。Objective To investigate the effect of the pretransplant intraportal administration of recipient blood to donor on rat hepatic allograft. Methods Male LEW (RT1l) and ACI (RT1a) rats were used as liver transplant recipients and donors,respectively and were divided into four groups as follows:group Ⅰ, no treatment; group Ⅱ, donors pre-treated by the intravenous administration of LEW recipient blood 7 days before transplantation; group HI, donors were pre-treated by the intraportal administration of third party BN blood 7 days before transplantation; and group IV,donors were pre-treated by the intraportal administration of LEW recipient blood 7 days before transplantation. Serum levels of interferon-γ(IFN-γ) were determined after transplantation. The number of dendritic cells of donor origin was measured with a monoclonal antibody against the rat RT1Ba class Ⅱ (1a) major histocompatibifity complex (MHC),OX76. Apoptotic hepatic infiltrates were determined. Results Animals in group Ⅰ survived a mean of 10.1±0.7 days. The survival time in group Ⅳ was prolonged significantly to 33.7±2.6 days. Serum concentration of IFN-γ increased significantly in group IV,as compared with group Ⅰ. The ratio of OX76^+ OX62^+ to OX62^+ cells was greater in group Ⅳ, as compared with group Ⅰ. Apoptotic hepatic infiltrates were more in group Ⅳ treated by the intraportal administration of recipient blood than those of group Ⅰ. Conclusion Pretransplant intraportal administration of recipient LEW blood to ACI donor rots prolongs the survival time of hepatic allograft. This may be associated with apoptosis of graft-infiltrating cells.
