摘 要:目的了解引起尿路感染的阴道加特纳菌的分离鉴定及其药敏状况。方法用经灭菌后的一次性定量采血管将清洁中段尿10μL接种到人血哥伦比亚琼脂,置35℃、5%CO2环境下孵育48 h后挑可疑菌落纯化,做API strep链球菌鉴定条和上PhoenixTM100仪鉴定细菌,确定为阴道加特纳菌后保存;菌株复苏后用琼脂稀释法测10种抗生素对54株阴道加特纳菌的50%最低抑菌浓度(MIC50)和90%最低抑菌浓度(MIC90)。结果所有54株阴道加特纳菌在人血哥伦比亚琼脂上均生长良好、菌落典型、β-溶血明显;药敏试验按厌氧菌判断标准,氨苄西林-舒巴坦、头孢西丁、头孢噻肟、头孢曲松、亚胺培南、克林霉素均100.0%敏感,其余依次为四环素(72.2%)、氨苄西林(68.5%)、左氧氟沙星(51.9%)、甲硝唑(35.2%)。结论引起尿路感染的阴道加特纳菌可用人血哥伦比亚琼脂分离培养,其所致尿路感染可用氨苄西林-舒巴坦、头孢西丁、头孢噻肟、头孢曲松、亚胺培南、克林霉素等治疗。建议尽快制订阴道加特纳菌的相应药敏试验标准,避免经验用药。Objective To explore the isolation and identification of Gardnerella vaginalis causing urinary tract infection and its drug-sensitivity condition. Methods 10 μL clean mid-portion urine was inoculated to human-blood Colombia agar culture medium by aseptic one-off quantitative measuring vessel. After 48 hours incubation in 35 ℃, 5% CO2, the suspicious bacteria picked up and purified was identified by API strep and Phoenix^TM 100. Gardnerella vaginalis was conserved after confirmation. 50% of the minimal inhibitory concentration ( MIC50 ) and 90% of the minimal inhibitory concentration ( MIC90 ) of 10 antibiotics against 54 isolates were tested by agar dilution method after the bacteria anabiosis. Results All 54 Gardnerella vaginalis isolates in human-blood Colombia agar culture medium grew well, and bacteria was representative and β-haematolysis was obvious. Drug-sensitivity test was performed according to the standard of anaerobic bacteria judgement. The sensitivity of ampicillin-sulbactam, cefoxitin, cefotaxime, ceftriaxome, imipenem and clindamycin were all 100, 0% , then tetracycline(72.2% ), ampicillin(68.5% ), levofloxacin(51.9% ), metronidazole (35.2%). Conclusions Gardnerella vaginalis which cause urinary tract infection could be isolated and cultured by human-blood Colombia agar culture medium. The infection could be cured by ampicillin-sulbactam, cefoxitin, cefotaxime, ceftriaxome, imipenem, clindamycin and others. It is important to establish the drug-sensitivity standard rule of Gardnerella vaginalis which could avoid treatment through experience.
