摘  要:目的探讨测血管内皮生长因子(VEGF)和血小板反应素1(TSP1)蛋白质在卵巢上皮癌中的表达与肿瘤微血管密度(MVD)的关系及其临床意义。方法用免疫组织化学法检测57例原发性卵巢上皮癌组织和22例正常卵巢组织中VEGF和TSP1蛋白质的表达,并用CD34抗体免疫染色后进行MVD计数。结合随诊资料,对上述指标与卵巢上皮癌临床病理学特征及3年存活率之间的关系进行回顾分析。结果卵巢上皮癌组织中VEGF蛋白质的阳性表达率高于正常卵巢组织(分别为70.2%和45.5%,P〈0.05),且多为强阳性表达。卵巢上皮癌中组织中TSP1多为弱阳性表达,总阳性表达率低于正常卵巢组织(分别为47.4%和72.7%,P〈0.05)。卵巢上皮癌组织的MVD显著高于正常卵巢组织(分别为30士9和20士8,P〈0.05)。VEGF阳性和TSP1阴性与卵巢上皮癌的手术病理学分期晚、大网膜转移和癌性腹水显著相关。VEGF的表达与MVD呈正相关(P〈0.01),TSP1的表达与MVD呈负相关(P〈0.01)。VEGF阳性和TSP1阴性均与患者3年存活率呈负相关。VEGF阳性而TSP1阴性组的3年存活率明显低于VEGF阴性而TSP1阳性组(分别为24.0%和83.3%,P〈0.01)。结论卵巢上皮癌组织VEGF和TSP1蛋白质的表达与肿瘤的血管形成有关,对上述分子的检测可帮助判断卵巢上皮癌的生物学行为,从而提供预后信息。Objective To evaluate the expression of vascular endothelial growth factor (VEGF) and thrombospodinl (TSP1) in epithelial ovarian cancer and their correlations with tumor angiogenesis and prognosis. Methods Fifty-seven samples of primary epithelial ovarian cancer and 22 samples of normal ovary from patients with hysteromyoma obtained during operation underwent immunohistochemistry with anti- VEGF, anti-TSP1, and anti-CD34 antibodies. Microvessel density (MVD) in the tumor tissue was evaluated on the base of CD-34 immunostaining. The correlations of VEGF and TSP1 expression with clinico-pathologic characteristics and 3-years survival rates were analyzed retrospectively. Results The positive rate of VEGF protein of the epithelial ovarian cancer was 70.2%, significantly higher than that of the normal ovarian tissue (45.5%, P 〈 0.05 ). The positive rate of TSP1 protein of the epithelial ovarian cancer was 47.4%, significantly lower than that of the normal ovarian tissue ( 72.7%, P 〈 0.05 ). The MVD of the epithelial ovarian cancer was (30 ± 9), significantly higher than that of the normal ovarian tissue ( 20 ± 8, P 〈 0. 05 ). The VEGF positive expression and TSP1 negative expression were positively correlated with the FIGO stage, omentum metastases, and malignant ascites ( P 〈 0. 01, 〈 0. 01, and 〈 0. 05 respectively). VEGF expression was positively correlated with MVD ( P 〈 0.01 ), and TSP1 expression was negatively correlated with MVD ( P 〈 0.01 ). Increased expression of VEGF and decreased expression of TSP1 with were reversely correlated with the patients' survival. The 3-year survival rate of the patients positive in VEGF and negative in TSP1 was 24.0%, significantly lower than that of the patients negative in VEGF and positive in TSP1 (83.3%, P 〈 0.01 ).
