补肾活血胶囊治疗老年男性骨质疏松症临床观察

摘　　要：目的:评价补肾活血胶囊治疗老年男性骨质疏松症的临床疗效和安全性,并探讨其作用机制。方法:入选老年男性骨质疏松症60例,平均年龄(73.65±5.6)岁,随机分为治疗组和对照组,每组30例。治疗组以补肾活血胶囊,对照组以钙尔奇+罗钙全治疗,服药1年。结果:补肾活血胶囊能显著提高老年男性骨质疏松症骨密度(P<0.05),明显改善其腰背疼痛(P<0.01)、下肢疼痛(P<0.01)、腰膝酸软(P<0.01)、下肢痿弱(P<0.05)、步履艰难(P<0.01)等临床症状,与对照组比较有极显著差异(P<0.01)。同时提高其骨钙素(P<0.01)、血清总雌二醇(P<0.05)、睾酮(P<0.05)及胰岛素样生长因子-1(P<0.01)水平,降低白介素-6(P<0.01)、尿吡啶酚(P<0.01)水平,与对照组比较显著意义(P<0.05或P<0.01)。治疗中无明显不良反应。结论:补肾活血胶囊通过改善老年男性骨质疏松患者的性激素水平和骨调节相关的细胞因子,改善其骨重建微环境,从而促进其骨形成,抑制其骨吸收,使骨密度显著提高,全身状态明显好转,综合效果良好。其治疗老年男性骨质疏松症安全,有效,无不良反应。Objective ：To investigate the therapeutic effect and safty of Bushenghuoxue capsules（BC） in aged male osteoporosis patients, and explore the regulating mechanism of BC on bone metabolism and bone remodeling of them. Methods ：The diagnosed osteoporosis patients were randomised into two groups- thirty patients in the control group, and so as in the treatment group. The treatment group took BC daily, and the control group took caltrate - D and rocaltrol daily for 12 monthes. The bone mineral density （BMD） of lumbarvertebra （L2 -4 ） and femoral neck were determined repectively before and after the treatment. Serum osteocalcin （ BGP）, testosterone（T） , estradiol （ E2 ）, interleukin - 6 （ IL - 6 ） ,insulin - like growth factor - 1 （ IGF - 1 ） and urine pyridinoline（PYD） were also measured. Results ：After the treatment the BMD（ P 〈0.05）,serum BGP（ P 〈0.01）,T（ P 〈0.05）, E2（ P 〈0.05） and IGF-1（ P 〈0.01）were significantly increased, and PYD（ P 〈0.01 ）, IL -6（ P 〈0.01） was significantyly decreased in the treatment group. The score of symptom and physical signs in the treatment group showed significant decrease after 12 months treatment（ P 〈0. 05 or P 〈 0.01 ） indicating significant change in physical signs and symptoms after treatment. Conclusion -By regulating the sex hormones and the bone regulating relevant cytokines in aged male patients with primary osteoporosis, BC can improve bone remodeling microenvironment, thus elevating the bone mineral density and promoting the quality of life. BC may effectively prevent and treat osteoporosis in old male patients.
