摘 要:目的:探讨腻滞质与正常质高血压患者动脉弹性与胰岛素抵抗间的相关性。方法:选择没有任何靶器官损害的1~2级高血压患者作为研究对象,其中腻滞质患者组198例和正常质患者组256例,使用CVProfilorDO-2020动脉弹性功能测定仪检测动脉弹性指数C1与C2,采用HOMA-IR公式计算胰岛素抵抗指数(IR),应用免疫透射比浊法测定超敏C反应蛋白(hsCRP);比较不同体质的高血压患者动脉弹性变化情况并对动脉弹性与胰岛素抵抗的相关性进行分析。结果:在对两组间的年龄和病程差异进行校正后,腻滞质组的C1、C2均明显低于正常质组(P<0.05),而腻滞质组的HOMA-IR及hsCRP明显高于正常质组(P<0.05)。单因素相关分析显示C2与胰岛素抵抗呈负相关;逐步回归分析显示,C2、体重指数(BMI)、hsCRP进入回归方程,与HOMA-IR独立相关。结论:腻滞质高血压患者动脉弹性降低较正常质患者更为明显,并且与患者的胰岛素抵抗密切相关。Objective: To investigate the correlation between the insulin resistance and arterial elasticity in the hypertensive patients of phlegm-dampness constitution and normal constitution. Methods: Degree Ⅰ and Ⅱ hypertension patients without target organ damage were enrolled in this study, including 198 cases of phlegm-dampness constitution and 256 cases of normal constitution. Pulse wave of arteria radialis and arterial elastic indexes C1, C2 were detected with CVProfilor DO-2020 arterial pulse contour instrument. Insulin resistance (IR) index was calculated with the HOMA-IR formula and the highly sensitive C reactive protein (hsCRP) was measured with immunoassay. The arterial elasticity of the phlegm-dampness constitution cases was compared with that of the normal constitution cases. The correlation between the arterial elasticity and insulin resistance in hypertensive patients was also analyzed. Results:After adjustment for ages and disease courses of the two groups, both C1 and C2 of the phlegm-dampness constitution group were lower than those of the normal constitution group evidently P〈0.05; HOMA-IR and hsCRP of the phlegm-dampness constitution group were much higher than those of the normal constitution group P〈0.05. Single factor correlation analysis indicated that there was significant negative correlation between C2 and insulin resistance. In the steply regression analysis, when C2, body mass index (BMI), hsCRP entered the regression equation, they were correlated with HOMA-IR independently. Conclusion: The decrease of arterial elasticity of phlegm-dampness constitution hypertensive patients is more evidently than that of normal constitution patients, which may correlate with the insulin resistance significantly.
