32例电视胸腔镜胸交感神经干切除术治疗手汗症报道

摘　　要：目的总结电视胸腔镜下胸交感神经切除术治疗手汗症的临床经验。方法分析2004年3月～2006年12月间，应用电视胸腔镜行选择性胸交感神经干切除术治疗32例手汗症患者的临床资料，并对该手术进行文献复习和讨论。结果32例患者手术全部成功。平均手术时间（双侧）42min（28～76min），平均住院时间4．8d。平均随访时间22．5个月（10～41个月），17例（53．1％）患者出现躯干、大腿和脚等处的代偿性多汗，但患者能够耐受，不需要进一步治疗。结论电视胸腔镜下选择性胸交感神经干切除术是治疗手汗症的一种疗效确切、创伤小和安全的治疗方法。Objective To summarize the clinical experience of video-assisted thoracoscopic sympathectomy for palmar hyperhidrosis. Methods From March 2004 to December 2006,32 patiens suffering from palmar hyperhidrosis underwent vldeo-assisted thoracoscopic sympathectomy. Their clinical materials were analysed retrospectively and the literatures were reviewed. Results The selective vldeo-assisted thoracoscopic sympathectomy was successfully performed in all patients. The operation time（both side） was 28-76 min with an average of 42 min. The median hospital stay was 4. 8 d（range： 3 to 5 d）. The mean follow-up was 22.5 months （range： 10 to 41 months）. Nine patients developed compensatory（increased） sweating in the trunk,5 patients in the thighs and the feet,and 3 patients in all these regions,but the symptoms were mostly tolerable and required no further treatment. Conclusion Video-assisted thoracoscopic sympathectomy is a safe, effective and minimally invasive therapeutic solution for patients with palmar hyperhidrosis.
