实时三维超声心动图检测左室重构指数评价冠脉搭桥手术效果

摘　　要：目的探讨实时三维超声心动图(RT3DE)检测左室重构指数来评价冠脉搭桥手术效果。方法48例研究对象分成正常对照组(A组)、搭桥3支组(B组)、搭桥4支及以上组(C组)。使用PhilipiE33和TomTec三维工作站,于手术前后采集心尖四腔Fullvolume图像,测量左室舒张末期容积(EDV)、收缩末期容积(ESV)、舒张末期心外膜轮廓容量(epiEDV),计算左室射血分数(EF)、左室质量(LVM)、左室重构指数(LVRI),并进行比较。结果B、C组中EDV、ESV、EF、LVRI手术前后比较差异具有统计学意义(P<0.05),LVM手术前后比较差异无统计学意义(P>0.05);B组术前各参数及术后EDV、ESV、EF与A组比较差异具有统计学意义(P<0.05),术后LVM、LVRI与A组比较差异无统计学意义(P>0.05);C组手术前后各参数与A组比较差异具有极显著统计学意义(P<0.01)。结论实时三维超声心动图通过全容积成像方式检测左室重构指数,能评价冠脉搭桥手术效果。Objective To evaluate the effect of coronary artery bypass grafting （CABG） with left ventricular remodeling index （LVRI） on real-time three -dimensional echocardiography （RT3DE）. Methods Forty eight patients were divided into normal control （GroupA）, three vessel grafted （Group B）, and four or more than four vessel grafted （Group C） . Left ventricular end-diastolic volume （EDV）, left ventricular end-systolic volume （ESV） and left ventricular end-diastolic epicardial volume （epiEDV） in all cases were measured with TomTec 4D LV analysis software offline on apical 4, 3 and 2 chamber views. Then, left ventricular ejection fraction （EF）, left ventricular enddiastolic mass （LVM） and LVRI were calculated pre-and post- CABG. Results There were significant differences of EDV, ESV, EF and LVRI between pre-and post- CABG （P〈0. 05 ）, but no significant difference of LVM was between pre-and post-CABG （P〉0.05） in Group B and C. There weresignificant differences of all parameters of pre- CABG and EDV, ESV and EF of post-CABG in GroupB, and all parameters of pre-and post-CABG in Groupc （P〈 0.05）, but no significant differences of LVM and LVRI of post-CABG in Group B, compared with Group A （P〉 0. 05）. Conclusions LVRI on RT3DE is a new index for clinical evaluation of CABG effects.
