摘 要:目的探讨厄贝沙坦对异丙肾上腺素(ISO)诱发的大鼠心肌肥厚-心力衰竭模型的左室肥厚和慢性心衰的干预作用。方法30只Wistar大鼠随机分成正常对照组、模型组、治疗组;通过大剂量注射ISO制成心肌肥厚-心力衰竭大鼠模型;治疗组应用厄贝沙坦对心肌肥厚-心力衰竭大鼠模型进行干预;各组大鼠于第10d、6周、12周行经胸超声心动图监测,并测定心肌和血浆血管紧张素Ⅱ(AngⅡ)水平。结果模型组大鼠舒张末期室间隔厚度(IVSTd)、舒张末期左室后壁厚度(LVPWd)增厚,左室腔扩大,与对照组比较差别有统计学意义(P<0.01);左室射血分数(EF)、左室短轴缩短率(FS)降低,与正常对照组比较差别也有统计学意义(P<0.01);治疗组经厄贝沙坦治疗后IVSTd、LVPWd有所减小,EF、FS有所提高,与模型组比较差别均有统计学意义(P<0.01)。模型组和治疗组大鼠血浆及心肌AngⅡ含量均较对照组高,差别有统计学意义(P<0.05);治疗组与模型组大鼠心肌及血浆的AngⅡ含量间差别无统计学意义(P>0.05)。结论造模大鼠的心室肥厚,心力衰竭与心肌局部和循环系统中肾素-血管紧张素-醛固酮系统(RAAS)过度激活有关。厄贝沙坦可通过作用于AngⅡ受体,拮抗心肌局部AngⅡ作用,改善心功能,逆转心肌肥厚。Objective To investigate the inventive effects of Irbesartan to left ventricular hypertrophy and chronic heart failure in rat models with myocardial hypertrophy and heart failure induced by isoproterenol (ISO). Methods Thirty Wistar rats were divided into normal control group, model group and therapy group randomly, with ten rats in each group. The rat models with myocardial hypertrophy and heart failure were established by subcutaneous injection of a larger dosage of ISO for 7 days, Rats in therapy group were given Irbesartan for intervention. All the rats in every group were monitored by echocardiograph via the thorax on the 10th day, 6th week and 12th week respectively. Myocardial and plasma angiotensin Ⅱ levels were measured in of rats in all groups, Results Compared with control group, the end -diastolic interventricular septal thickness (IVSTd) and left ventricular posterior wall thickness (LVPWd) in the model group were increased and the left ventricular cavity enlarged ( P 〈 0. 01 ) ; the ejection fraction (EF) and fractional shortening (FS) were decreased ( P 〈 0. 01 ). Compared with model group, the IVSTd and LVPWd in the therapy group were reduced and the EF and FS were increased, ( P 〈0. 01 ), The myocardial and plasma angiotensin Ⅱlevels in both the model and therapy groups were obviously higher than those of the control group ( P 〈 0. 05 ), but no significant difference was found between those of the therapy group and those of the model control group ( P 〉 0.05 ), Conclusion The ventricular hypertrophy and heart failure of the model rats are related with over activity of Renin - Angiotensin - Aldosterone System in local myocardia and circulatory system. Irbesartan could improve cardiac function and reduce the left ventricular hypertrophy by antagonizing the actions of angiotensin Ⅱin cordial tissue and plasma through angiotensin Ⅱ receptors.
