40层螺旋CT冠状动脉成像在诊断冠状动脉疾病中的应用

摘　　要：目的评价40层螺旋CT冠状动脉成像的效果。方法将68例临床疑诊冠心病患者及支架植入术后患者心率控制在70次/min以后采用ECG门控扫描及图像重建技术,应用Philips Brilliance40层螺旋CT先行前瞻性心电门控平扫后,回顾性心电门控增强扫描,对扫描数据在工作站行VR、CPR、MIP等后处理,得到冠脉主要分支图像,并根据图像来评价冠状动脉的行径、管腔狭窄情况及支架内通畅情况等。结果心率在70次/min以下的图像清晰完整,68例患者明确显示了冠状动脉主干及分支的管腔情况,其中发现了左前降支钙化伴狭窄20例,左室陈旧性心肌梗死3例,支架植入术后管腔通畅5例,肺动脉圆锥支起源于右冠状动脉窦15例。结论Philips Brilliance40层螺旋CT较高的空间分辨率和扫描速度已完全能满足临床诊断冠心病的需要,可首选为非创伤性冠状动脉检查。Objective To evaluate clinical value of 40 slices spiral CT coronary artery imaging for diagnosis of coronary artery disease. Methods Total of 68 patients who were suspected to be coronary heart diseases or received stent embedding were enrolled in this study. The heart rate was controlled less than 70 times per minute. ECG-triggered scanning and image-rendering technique were applied. After prospective ECG-gating scanning ,the patients were given retrospectively ECG-gating contrast scanning. Data were post-precessed by VR,CPR,MIP, etc. in the workspace. According to the imaging of major branches of coronary arteries the behavior of coronary artery,luminal stenosis and the state of implanted stent were evaluated. Results The imagings less than 70 times per minute of heart rate were clear and complete. The lumens of coronanry arteries were clearly demonstrated. Calcification and stenosls of left anterior descending branch （20 cases）, old myocardial infarction （3 cases） ,unobstructed lumens of stent embedding（5 cases） and pulmonary infundibulum branches originating from right coronary sinus（ 15 cases） were found. Conclusions The 40 slices spiral CT is much help to the diagnosis of coronary artery disease and can be the first choice of noninvasire examination of coronary artery for its high spatial resolution and scanning speed.
