摘  要:目的探讨还原型谷胱甘肽(GSH)对多发伤后肝功能再损伤的预防作用。方法选择多发伤后需再次手术的患者69例,随机分为预防组(33例)和对照组(36例),预防组再次手术前3 d和术后4 d给予GSH1.8 g/d静脉滴注。给予GSH前及术后第5 d查总胆红素(TBiL)、直接胆红素(DBiL)、丙氨酸氨基转移酶(ALT)、天冬氨酸氨基转移酶(AST)、碱性磷酸酶(ALP)、谷氨酰转肽酶(GGT)和胆碱脂酶(CHE),并观察疗效,进行两组手术前后肝功能指标和疗效比较。结果再次手术后预防组和对照组的ALP、GGT、CHE比较差异无统计学意义(P>0.05),而两组的TBiL、DBiL、ALT、AST间差异有统计学意义(P<0.05);两组疗效间差异有统计学意义(P<0.05)。结论还原型谷胱甘肽对肝功能的再损伤有预防作用。Objective To research clinical preventive effect of reduced glutathione (GSH) on the reinjury of multi - injured liver function. Methods 69 cases, who needed reoperation after multi - injury of liver, were randomly divided into a preventive group ( n = 33 ) and a control group ( n = 36). The preventive group were given Ⅳ drops of GSH ( 1.8 g/d) for 3 days before the operation and for 4 days after it. We examined the total bilirubin (TBIL), direct bilirubin ( DBIL), alanine aminotransferase (ALT), aspartate aminotransferase (AST), glutamyltransferase (GGT), alkaline phosphatase (ALP), cholinesterase (CHE) before using GSH and on the 5th day after surgery, and compared the liver functional parameters and total effective rates before operation with those after it. Results There was significant difference in TBIL、 DBIL、 ALT and AST between the two groups after the re - operation ( P 〈 0. 05 ), while no significant difference was found in CHE 、 GGT and ALP between them ( P 〉 0. 05 ). The total efficacy ratios were respectively 84. 85 % ( 28/33 ) and 44. 4 % (16/36). There was significant difference in curative effect between the two groups ( P 〈 0. 05 ). Conclusion GSH plays a role in prevention of re - injury of liver function.
