复方氨基酸致过敏性休克8例分析

摘　　要：目的观察8例复方氨基酸致过敏性休克患者的临床特点,探讨其防治方法。方法总结2005年5月-2006年8月8例注射复方氨基酸致过敏性休克患者的适应证、过敏史、临床表现、实验室检查和诊断、治疗的特点。结果8例患者中有6例没有明确的用药适应证,患者用药前均未被明确询问氨基酸过敏史。患者临床表现高热、畏寒、低血压并可持续达4 d,患者外周血白细胞数均明显升高,经肾上腺素等升血压、异丙嗪及糖皮质激素抗过敏治疗后均好转出院。结论复方氨基酸在农村基层应用较多,但应严格掌握其适应证,用药前明确询问并记录患者过敏史。开始用药时速度相对要慢,出现畏寒、寒战应立即停药,并严密观察患者的血压变化,监测时间相对要长,治疗上早期应用抗过敏药物。Objective To observe the clinic features of anaphylactic shock due to compound amino acid transfusion to approach the methods of treatment and prevention. Methods We reviewed the data of the clinical features of drug indication, history of anaphylaxis, clinical manifestation, lab tests, and diagnosis and treatment in 8 patients with anaphylactic shock due to transfusion of compound amino acid （ from 2005 - 05 - 2006 - 08 ）. Results All patients were not inquired about their anaphylactic history of amino acid before taking drugs, 6 among whom had not clear drug indication. They manifested clinically high fever, chill and hypotension which lasted as long as 4 days with ascended white blood cell. All the patients took a favourable turn and were discharged after using adnephrin to elevate blood pressure, and anti - anapgylaxis treatment of diprazine and glucocorticold. Conclusion Compound amino acid is widely used in the countryside. Its clear drug indication should be deeply concerned, and patients should be inquired about their history of anaphylaxis. When it is transfused, the velocity should be controlled within a low range at the beginning; If patients feel chilly, transfusion must be stopped, and their blood pressure should be observed, and anti -anaphylaxis drugs should be used soon.
