AO外固定架治疗重度开放性胫腓骨骨折的疗效分析

摘　　要：目的观察AO外固定架治疗重度开放性胫腓骨骨折的疗效。方法54例重度开放性胫腓骨骨折均行同样的合并伤的治疗方法。彻底清创术,以后根据患者的意愿分为两组,治疗组29例AO外固定架治疗,对照组25例采用支撑钢板内固定治疗,以骨折复位质量、骨愈合时间及并发症三个方面进行比较分析。结果治疗组和对照组两种固定方法的统计学意义比较,治疗组优率、良率和差率分别为79.31%、13.79%和6.90%,而对照组相应率分别为48%、20%和32%,经比较,差异有统计学意义(X^2=6.910,P:0.032<0.005);临床意义分析,RR=0.2156<1,RRR=0.7844>0,ARR=0.251>0,对患者有利。NNT=3.984,表示为了使1个患者免于疗效差,需应用AO外固定架治疗4个患者。结论AO外固定架治疗重度开放性胫腓骨骨折无论从统计学意义比较,还是临床意义分析,均疗效高,有利于患者。Objective Observes AO soildly outside to decide the treatment specific weight open shin fibula bone fracture the curative effect. Methods 54 example specific weight open shin fibula bone fracture line of similar merge wound method of treatment. The thorough epluchage, later will divide into 2 groups according to patient＇ s wish, outside treatment group 29 example AO the bracket fixing treatment, the control group 25 examples uses supports in the steel plate the fixed treatment, by the bone fracture replacement quality, the bone cicatrization time and the complication 3 aspects will carry on the comparative analysis. Results The treatment group and the control group two fixed method statistics significance comparison, the treatment group superior rate, good rate and the error rate respectively are 79.31%, 13.79% and 6.90 %, but the control group corresponding rate respectively is 48%, 20 % and 32 %, after the comparison, the difference has statistics significance（x2 = 6.910, P = 0. 032 〈 0. 005） ; clinical significance analysis RR = 0. 2156 〈 1, RRR = 0. 7844 〉 0, ARR = 0. 251 〉 0, is advantageous to the patient. NNT = 3. 984, the expression in order to cause 1 patient to be bad avoid the curative effect, must apply outside AO the bracket fixing to treat 4 patients. Conclusion Outside conclusion AO bracket fixing treatment specific weight open shin fibula bone fracture regardless of from statistics significance comparison, the clinical significance analysis, the even curative effect is high, is advantageous to the patient.
