摘 要:目的观察长期低浓度一氧化碳(CO)作业对工人健康状况的影响。方法燃气作业人员427名为长期慢性接触CO接触组,无接触CO人员560名为对照组。2组体检结果进行比较。内容包括:脉搏、血压、谷丙转氨酶(ALT)、谷草转氨酶(AST)、总胆固醇(TC)、甘油三脂(TG)、高密度脂蛋白(HDL-C)、低密度脂蛋白(LDL-C)、肌酐(CR)、尿素氮(BUN)、尿酸(UA)、空腹血糖(GLU)、血常规、尿常规、胸部X线检查、心电图、腹部B超等。结果接触组的收缩压(SBP)和舒张压(DBP)水平分别为106.88±13.60mmHg、69.96±10.35mmHg,对照组的SBP和DBP水平为103.33±14.77mmHg、68.10±10.59mmHg,接触组的血压水平明显高于对照组(P<0.01)。接触组的CR、BUN、UA、GLU、TC、TG、HDL-C和LDL-C水平分别为62.88±12.85μmol/L、3.86±1.22mmol/L、306±94μmol/L、5.32±1.44mmol/L、4.56±0.84mmol/L、1.35±0.82mmol/L、1.21±0.30mmol/L和2.59±0.68mmol/L。对照组的CR、BUN、UA、GLU、TC、TG、HDL-C和LDL-C水平分别为67.87±16.12μmol/L、4.93±1.31mmol/L、328±88μmol/L、5.11±1.45mmol/L、4.64±0.85mmol/L、1.16±0.77mmol/L、1.39±0.33mmol/L和2.69±0.76mmol/L。接触组的CR、BUN、UA、HDL-C和LDL-C水平低于对照组(P<0.05);GLU、TG水平则高于对照组(P<0.05)。接触组的脂肪肝、空腹血糖受损(IFG)、心电图异常的患病率分别为19.91%、19.67%、18.74%,高于对照组(P<0.05),泌尿系统结石的患病率为10.30%,低于对照组(P<0.05)。结论长期低浓度一氧化碳作业对工人的心血管系统,肾功能,空腹血糖和血脂代谢有明显的影响。Objective To investigate influence of long-term exposure to low-dose carbon monoxide on workers' health. Methods A total of 427 workers exposed to carbon monoxide were chosen as example group and 560 workers without exposure to carbon monoxide as control group. Data of the 987 workers, including pulse, blood pressure, ALT, AST, TC, TG, HDL-C, LDL-C, CR, BUN, UA, GLU, blood routine test, urine routine test, chest X-ray, ECG, abdominal ultrasound and so on were collected and compared. Results In example group, the levels of SBP, DBP were 106.88± 13.60 mmHg, 69.96 ±10. 35 mmHg respectively, while in control group, which were 103.33 ±14.77 mmHg, 68.10 ± 10.59 mmHg respectively. The average blood pressure level in example group was significantly higher than that in control group ( P 〈 0.01 ). In example group, the levels of CR, BUN, UA, HDL-C, LDL-C were 62.88± 12.85 μmol/L, 3.86± 1.22 mmol/L, 306 ±94μmol/L, 1.21±0. 30 mmol/L, 2.59±0.68 mmol/L respectively. They were lower as compared with those in control group (P 〈0.05) , which were 67.87 ± 16. 12μmol/L, 4. 93± 1.31 mmol/L, 328±88 μmol/L, 1.39±0. 33 mmol/L, 2.69±0. 76 mmol/ L respectively. But the levels of GLU, TG were higher in example group than that in control group (5.32±1.44 mmol/L) vs (5.11±1.45 mmol/L), (1.35±0.82 mmol/L) vs ( 1.16±0.77 mmol/L) (P 〈0.05). Prev-alence of fatty liver, IFG, ECG abnormality were different between two groups, and they were higher in example group (P 〈 0. 05 ). Prevalence of urinary stone in example group was lower than that in control group (P 〈 0. 05). Conclusion Long-term exposure to low-dose carbon monoxide can obviously affect cardiovascular system, renal function, blood glucose and lipid metabolism of workers.
