运动试验中ST/HR斜率及ST段延长与冠状动脉病变的相关性研究

摘  要:目的探讨运动心电图中ST/HR斜率、ST段延长与冠状动脉病变支数及冠状动脉狭窄程度的相关性。方法112例同期进行B ruce方案平板运动试验和冠状动脉造影且确诊的缺血性心脏病48例,对其ST/HR斜率、ST段延长、冠状动脉病变支数、冠状动脉狭窄程度测量,并进行相关及多元线性回归分析。结果冠状动脉病变支数与最大ST/HR斜率和ST段延长呈线性正相关,相关系数为r=0.67,r=0.559,P均<0.0001。冠状动脉狭窄程度与最大ST/HR斜率和ST段延长呈线性正相关,相关系数分别为r=0.799,r=0.513,P均<0.0001。结论最大ST/HR斜率及ST段延长对判断冠状动脉病变支数、冠状动脉狭窄程度都有临床价值,用最大ST/HR斜率比用ST段延长值判断冠状动脉病变支数更敏感,以最大ST段延长值比用最大ST/HR斜率值判断冠状动脉狭窄程度更敏感。Objective To discuss the correlation of ST/HR slope or S -T prolongation in EET with branch and narrow area of the diseased coronary artery vessel. Methods 112 patients with the probability of IHD were received treadMill exercise electrocardiography testing under Bruce protocol and coronary arteriongraphy. According to coronary arteriongraphy, 48 cases were diagnosed IHD and then mmeasure their ST/HR slope, S - T prolongation, branch and narrow area of the diseased coronary artery vessel, adopt analysis coefficient of multiple linear correlation. Results The study were analyzed by maximum linear regression analysis: firstly selected branchs of the diseased coronary artery vessels as dependent, ST/HR slope and ST prolongation as independent, ST/HR slope enter mode, ST prolongation didnt enter. Secondly selected narrow area of the diseased coronary artery vessels as dependent, ST/HR slope and ST prolongation as independent, ST prolongation enter model. Conclusion ST/HR slope's adherence is better than ST prolongation in diagnosis of branchs of the diseased coronary artery vessel. However, ST prolongation's adherence is better than ST/HR slope in diagnosis of degree of coronary artery stenosis.
