摘  要:目的探讨超声引导胶体磷酸铬(32P胶体)注射治疗巨大甲状腺囊肿的临床应用价值、安全性及对甲状腺功能的影响。方法34例孤立甲状腺囊肿患者分别在超声引导下给予32P胶体18.5-37 MBq/ml囊腔内注射治疗,治疗前、治疗后3个月分别进行甲状腺B超检测,计算囊肿体积,测定游离T3(FT3)、游离T4(FT4)、促甲状腺激素(TSH),比较治疗前、治疗后3个月甲状腺结节体积、FT3、FT4、TSH变化。结果治疗总有效率91.18%,治愈率及显效率79.30%。治疗后3个月甲状腺结节体积(3.73±2.15)cm3较治疗前(14.18±7.83)cm3显著缩小(P<0.05);治疗前与治疗后3个月FT3,FT4,TSH比较无统计学差异(P>0.05)。结论超声引导32P胶体注射治疗较大孤立甲状腺囊肿疗效确切,操作简便,创伤小,安全性高,近期内对甲状腺功能无影响。Objective To evaluate the clinical value and safety of injection of radioactive colloidal chromium phosphated-32 (^32P) in treatment of thyroid cyst. Methods Thirty-four patients with thyroid cyst were treated with injection of ^32^ ( 18.5 - 37 MBq/ml) in cyst under the guidance of ultrasound. The sizes of cysts were defined by ultrasound scan before treatment and after treatment for 3 months. The volumes of thyroid nodules were calcNated by formula V = length width × height × π/6. Free T3 ( FT3 ), free T4 (FT4) and thyrotropin releasing hormone (TSH) were determined before treatment and after 3 month treatment. Results The total effective rate was 91.18%. The curative rate and markedly effective rate together was 79.30%. Compared with that before treatment, the size of thyroid nodules significantly decreased after 3 month treatment [ ( 14.18 ± 7.83)cm^3 vs (3.73 ± 2.15)cm^3, P 〈 0.05 ]. While FT3 , FT4 and TSH had no significant difference before and after 3 month treatment(P 〉0.05). Conclusion The therapy with radioactive colloidal chromium phosphated-32 by injection into cyst in the patients with thyroid cyst is effective. The thyroid's function would not be affected over a short period of time.
