重症急性胰腺炎大鼠HMGB1表达与肠黏膜屏障损害的关系

摘　　要：目的:观察大鼠重症急性胰腺炎(severe acute pancreatitis,SAP)时肠组织中高迁移率族蛋白B1(high mobility group box-1protein,HMGB1)表达的变化及其与肠黏膜屏障损害的关系。方法:48只Wistar大鼠随机分为正常对照组(n=8)和SAP组(n=40)。正常对照组麻醉后取材,SAP组分别于建模后3、6、12、24和48h取材。测定血浆D-乳酸、肠组织髓过氧化物酶(myeloperoxidase,MPO)水平,应用RT-PCR方法检测SAP大鼠肠组织中高迁移率族蛋白B1 mRNA表达的变化,应用western blot法检测SAP大鼠肠组织中HMGB1水平的变化。结果:随着SAP病情进展,血浆D-乳酸与肠组织MPO水平在24h达最高值,分别为(16.41±4.65)μg/mL和(26.76±3.63)U/g(P<0.01)。SAP 6h时大鼠肠组织HMGB1 mRNA及HMGB1表达水平显著高于对照组,并于24h达峰值且持续至48h(P<0.01)。结论:SAP大鼠肠组织中HMGB1表达延迟且持续增高。HMGB1作为晚期炎症介质参与了SAP大鼠肠黏膜屏障损伤的病理生理过程。Objective：To investigate the relationship between high mobility group box-1 protein （HMGB1） expression and gut mucosal barrier dysfunction during murine severe acute pancreatitis （SAP）. Methods：Forty-eight male health adult Wistar rats were divided randomly into Control group and SAP groups. The concentration of plasma D-lactate and the activity of myeloperoxidase （MPO） in the intestinal tissue were determined. The expression of HMGB1 mRNA in intestinal mucosa was detected by reverse transcription polymerase chain reaction （RT-PCR） and the activity of HMGB1 was determined by Western blot. Results：Plasma D-lactate and MPO reached a peak level at 24h （16.41±4.65）μg/mL for Plasma D-lactate and（26.76±3.63）U/g for MPO respectively, （P〈0.01）. Elevation of HMGB1 and HMGB1 mRNA expression in intestinal mucosa was found at 6 hour after SAP, HMGB1 and HMGB1 mRNA expression peaked at 24 hours and kept relative high values up to 48 hour compared with normal control group （P〈0.01）. Conclusion：The rise of HMGB1 expression might play an important role in the intestinal mucosal barrier injuries in SAP. HMGB1 as a late mediator was involved in the pathogenesis of SAP.
