摘 要:目的:研究创伤痛应激所致的免疫抑制及其昼夜节律变化,以及针刺抗创伤痛所致免疫抑制的效应是否也具有昼夜节律。方法:选择经过痛阈筛选择的192大鼠,将动物随机分为24组,每组8只,其中每4组作为1个观察时间点,每组8只动物,分别于6个相应时间点对动物进行处理:空白对照组(不截肢,不电针)、空白针刺组(不截肢,仅电针)、截肢模型组(只截肢,不电针)、截肢电针组(截肢,同时处以电针足三里、环跳穴),处理结束后24 h在相应时间点处死动物,采集样本,用放射免疫法测电针对截肢大鼠血浆抗炎/促炎细胞因子(IL-10/IL-6)比值的昼夜节律的影响。实验时同一时间点的4个组的手术、电针及动物处死、取样均同时进行。结果:针刺能提高正常大鼠IL-10/IL-6比值,截肢后比值明显降低,截肢时电针能升高IL-10/IL-6比值,但仍较正常组低。从节律方面看,空白对照组血浆IL-10浓度具有一定的昼夜变化趋势,电针后IL-10浓度有所下降,仍具有明显昼夜波动;截肢后血浆IL-10浓度明显下降,昼夜节律不明显;截肢电针组血浆IL-10浓度较空白对照组、空白针刺组有所下降,但较截肢模型组高。空白对照组血浆IL-6浓度呈现双峰结构的变化趋势,电针后IL-6浓度有所上升,变化趋势不规律,表现为波动不大,未见明显的昼夜节律变化;截肢后血浆IL-6浓度明显升高,出现明显昼夜节律,其最大值出现在下午16时左右,最小值值出现在凌晨4点左右;截肢时电针,动物血浆IL-6浓度较截肢模型有轻度下降。结论:电针对血浆IL-10/IL-6具有双向调节作用。针刺作为一种较轻微的应激刺激,对正常动物的免疫系统功能有一定影响,且能对抗创伤痛所致的免疫抑制,且这种抗免疫抑制效应具有一定的昼夜节律。Abstract: Objective: To understand the circadian rhythm of electro - acupuncture efficacy in the treatment of immune suppression, the variability of electro - acupuncture efficacy at different time points in a 24 - hour period. Methods: Rats (192) with pain threshold exam bolting were, without prejudice, divided into 24 groups with 8 in each group. For the subsequent experiments, four groups were used for each time point, and six different time points were then chosen as8:00 (ZTO), 12:00 (ZT4), 16:00 (ZT8), 20:00 (ZT12), 24:00 (ZT16) and4:00 (ZT20). Rats were then subject to different treatments, (1), no amputation (AMP) and electro -acupuncture (Control group) ; (2), no AMP, but with electro -acupuncture on points ST36 and GB30 (Electro -acupuncture Group); (3), AMP but no electro- acupuncture (AMP Group); AMP and electro- acupuncture treatment on points ST36 and GB30 (AMP -Electro -acupuncture Group). At each time point, tail -flick method was employed to evaluate the pain threshold. The plasma method. Results: The ratio of plasma anti levels of IL - 10 and IL - 6 were determined by using radio - immunity - inflammatory to pro - irfflamatory cytokine levels ( IL - 10/IL - 6 ) in rats of control group has a obvious circadian rhythm. As immune suppression results from traumatic pain, electro - acupuncture has lesser effects on the rats' immune system in Electro - acupuncture Group and therefore has greater various regulatory effects at different time points. When compared with the AMP group, most of rats in AMP - electro - acupuncture group showed increasing IL - 10 levels at 8 am and decreasing IL - 6 levels on 8pm. The most marked effects of electro - acupuncture on the ratios of IL - 10/IL - 6 were observed at 8am and 8pro. Further, electro- acupuncture could raise the thymus gland exponent of insulted rat to different extent at different time points showing the biggest increase at 12am and 8pm.
