彩色多普勒超声对前置血管的诊断及临床价值

摘　　要：目的:评价彩色多普勒超声(CDFI)对前置血管的诊断价值及临床意义。方法:回顾分析19例前置血管彩色多普勒超声的声像图表现及母儿结局,并进行超声分型分析。结果:16例前置血管患者的彩色多普勒超声诊断与临床和病理诊断结果一致,诊断符合率84.2%;漏诊3例(占15.8%)。19例前置血管患者中彩色多普勒超声检出脐带帆状附着型10例,0副胎盘型4例,双叶胎盘型3例,脐带胎盘边缘附着型2例。19例前置血管孕妇剖宫产16例,阴道分娩3例;活婴23例,死胎7例;母亲全部康复出院。结论:彩色多普勒超声对前置血管的诊断准确率较高,能降低围生儿死亡率,可作为前置血管的首选检查方法。Objective： To evaluate the diagnosis and clinical value of color doppler flow imaging （ CD- FI ） in the treatment of vasa previa. Method： A retrospective analysis was made on the CDFI ultrasonographical features of 19 cases patients with vasa previa and also made on the situation of mother and baby, then carrying out ultrasound classification. Result： There were 16 cases vasa previa patients whose CDFI diagnosis was in conformity with the clinical and pathological diagnosis. The coincidence rate was 84.2%. There were 3 cases patients who belonged to missed diagnosis （ 15.8% ）. Of 19 cases patients with vasa previa checked by CDFI, 10 cases were velamentous insertion of umbilical cord, 4 cases were accessory placenta, 3 cases were placenta bipartite and 2 cases were marginal insertion of umbilical cord. Of 19 cases pregnant women with vasa previa, 16 cases were conducted cesarean birth and 3 cases gave birth by natural labor, including living babies of 23 cases and stillbirths of 7 cases. All mothers recovered and left the hospital. Conclusion： It is accurate that the vasa previa is diagnosed by CDFI, which can drop the death rate of fetus. Therefore, it shall be the first -choice method in the examination of vasa previa.
