摘　　要：目的:探讨12导动态心电图与12导常规心电图ST-T的差别,为提出12导动态心电图(AECG)ST-T的诊断标准提供依据。方法:用常规12导心电图(RECG)的连接方式和12导动态心电图的连接方式分别描记心电图,然后比较两种连接方式心电图ST?T的差别。结果:两种导联联接方式,ST段正常或改变基本一致的555例,一致率为92.5%,但AECG的ST改变较RECG显著;T波正常或改变基本一致的561例,一致率为94.8%(T波的一致是指:在QRS波群主波向上的导联,T波方向一致,T波的电压变化均在正常范围内,形态大致相同);Q-Tc间期:两种连接方式均在正常范围内586例,一致率为97.5%。结论:①两种联接方式,虽然ST段的一致率为92.5%,但AECG的ST改变较RECG显著,所以AECG的ST诊断标准应比RECG的ST诊断标准更为严格;②两种联接方式T波的一致率为94.8%差别较ST段小,在分析12导联AECG T波时,可适当考虑这一情况;③两种联接方式Q-Tc间期一致率为97.5%,可考虑使用同一诊断标准。Objective： To explore the difference of ST -T between 12 -lead dynamic electrocardiogram （ DCG） and 12 - lead routine electrocardiogram （ RECG） , then to investigate the diagnostic standard of ST - T by DCG. Method：600 patients were detected with DCG and RECG respectively, and the difference of ST T was analysed. Result： The changes of ST- T in 555 cases detected by DCG were in accordance to those by RECG . The concordance rate of ST - T detection was 92.5% . But ST - T variation by DCG was more significant than that by RECG. 561 patients show a normal T - wave or similar changes in T - Wave by both DCG and RECG. The concordance rate was 94.8% . The Q - Tc intervals of 586 cases were all in normal range by both DCG and RECG. The concordance rate of Q -Tc interval detection was 97.5%. Conclusion： Firstly, the diagnostic standard of ST by DCG maybe sensitive than that by RECG. Secondly, The concordante rate of T - wave detection was higher than that of ST - T. Lastly, Identical standard can be used for Q -Tc interval diagnosis by DCG and RECG since a high concordance rate of Q -Tc interval was found.
