新疆维吾尔族霍奇金淋巴瘤与EB病毒感染的关系

摘　　要：目的探讨新疆地区维吾尔族霍奇金淋巴瘤与EB病毒感染的关系。方法对52例病理确诊为霍奇金淋巴瘤(Hodgkin's lymphoma)的维吾尔族病例用免疫组化检测EBV,并对其中的27例用原位杂交检测EBV编码的mRNA(Epstein-Barr virus encoded mRNA,EBER)。结果EBV:52例HL中40例阳性(76.9%),其中结节硬化型23例,混合细胞型10例,富于淋巴细胞型5例,淋巴细胞减少型2例。EBV在≤14岁的儿童中阳性率为95%(19/20),在14岁以上年龄组阳性率为65.5%(21/32)。EBER:22例阳性,阳性率为81.5%(22/27),其中结节硬化型10例,混合细胞型8例,富于淋巴细胞型4例,无淋巴细胞减少型。EBV在≤14岁的儿童中阳性率为88.9%(8/9),在14岁以上年龄组阳性率为77.8%(14/18)。结论新疆维吾尔族霍奇金淋巴瘤中有高水平EBV感染的现象,尤其是儿童;霍奇金淋巴瘤各亚型的EBV感染率无明显差异。Purpose To investigate the association between Hodgkin＇s lymphoma and Epstein Barr virus （EBV） infection in Uygur, Xinjiang. Methods 52 cases of pathologically diagnosed Hodgkin＇s lymphoma were collected and EBV was detected with immunohisto- chemistry in all cases. In addition,Epstein Barr virus encoded mRNA （EBER） was also detected by using in situ hybridization in 27 of the 52 cases. Results Forty of the 52 cases （76.9%）were showed EBV positive by using immunohistochemistry, including 23 nodular sclerosis （23/29）, 10 mixed cellularity （10/13）,5 lymphocyte rich （5/8）, and 2 lymphocyte depletion （2/2）. The positive rate of EBV was 95% （19/20） in those younger than 14 years and 65.5% （21/32）in those older than 14 years. The frequency of EBER was 81.5% （ 22/27 ） with in situ hybridization, including 10 nodular sclerosis （10/12） ,8 mixed cellularity （ 8/8 ） ,4 lymphocyte rich （4/ 6） ,and none of lymphocyte depletion （0/1）. The incidence of EBER was 88. 9% （8/9） in those younger than 14 years and 77. 8% （ 14/18 ） in those older than 14 years. Conclusion Hodgkin＇s lymphoma in Uygur, Xinjiang was associated with EBV infection, especially in children. However,incidence of EBV infection has not significant difference among the subtypes of Hodgkin lymphoma.
