长期运动训练使慢性稳定型心力衰竭患者标准药物疗效更佳

摘　　要：目的:评价长期中等强度运动联合标准药物治疗心力衰竭病人的效果。方法:药物标准治疗的30例心力衰竭病人随机分为两组:1年运动训练组(ET,Ⅰ组,n=15),和非运动训练组(Ⅱ组,n=15)为对照组。运动前、后测6min步行试验和检查超声心动图。结果:1年后,两组的左室舒张末期内径(LVEDd)明显减少[Ⅰ组:自(71.1±9.7)mm降至(61.0±6.6)mm;Ⅱ组:自(69.1±9.5)mm降至(64.8±8.1)mm],两组的LVEF和6min步行试验结果明显改善[Ⅰ组:LVEF自(30.8±9.9)%升至(48.0±10.1)%;6min步行距离:自(378±81)m升至(545±96)m;Ⅱ组:LVEF自(29.9±8.7)%升至(40.7±7.2)%;6min步行距离:自(369±89)m升至(463±101)m](P均<0.05)。同时,与对照组相比,运动训练组的LVEDd明显减少,而LVEF和6min步行距离也明显增加(P均<0.05)。结论:长期适当的运动训练可使心力衰竭病人在标准药物治疗获益的基础上进一步改善左室重构和收缩功能,增加6min步行距离。Objective： To evaluate the effects of long-term moderate intensity exercise training （ET） combined with medicine therapy in patients with congestive heart failure （CHF）. Methods： The 30 CHF patients were randomly assigned to a 1-year exercise training group （G1, n=15） and non-exercise training group as a control （G2, n=15） . 6 rain walking test and echocardiography were measured in all patients before and after ET. Results： After 1 year, left ventricular end-diastolic dimension （LVEDd） diminished markedly both in G1 and G2 （control group） [GI： from （71.1±9.7） mm to （61.0±6.6） mm and G2： from （69.1±9.5） mmto （64.8±8.1） mm] （P〈0.05）. Left ventricular ejection fraction （LVEF） and 6 rain walking distance improved significantly both in two group [in G1, LVEF from （30.8±9.9）% to （48.0±10.1）%, walking distance from （378±81） mto （545±96） m, in G2, LVEF from （29.9±8.7）% to （40.7±7.2）% and walking distance from （369±89） m to （463±101） m] （P〈0.05 all）. Furthermore, LEVDd markedly decreased and LVEF, 6 rain walking distance significantly increased in G1 group compared to control group （P〈0.05 all）. Conclusion： Long-term exercise-training produces a more benefit effects in the heart failure patients with standard medicine therapy.
