三种检测系统测定血清电解质的方法学对比与评估

摘　　要：目的:对Olympus Au-2700全自动生化分析仪、迅达XD-685电解质分析仪和美国强生VITROS350全自动干式生化仪3种血清电解质(K+,Na+,Cl-)测定方法进行对比与偏差评估。方法:依据美国国家临床实验室标准化委员会EP9-A2文件,每天选取临床标本8份,分别用3种检测系统测定标本K+,Na+,Cl-含量,共测定5天,记录检验结果,检查离散点,计算线性方程和相关系数(r),并对其进行偏倚评估。结果:Olympus Au-2700与VITROS350测K+,Na+,Cl-的r分别为0.999,0.987和0.982,Olympus Au-2700与XD-685测K+,Na+,Cl-的r为0.999,0.983和0.975,VITROS 350与XD-685与测K+,Na+,Cl-的r为0.999,0.987和0.986,三者比较的相关性都较好,临床可接受性评价均未超过CLIA′88允许误差的一半。结论:XD-685,Olympus Au-2700和VITROS350 3种检测系统测定血清电解质K+,Na+,Cl-结果基本一致。同一实验室的检测系统应进行方法对比和偏差评估从而判断其临床可接受性以保证检验结果的可比性。Objective： To evaluate the results of electrolyte assay by using three automatic biochemistry analyzer systems： Johnson Olympus Au-2700, Vitros-350 and XD-685. Methods： According to the protocol described by the NCCLS document EP9-A2, 8 Clinical samples were taken each day and the serum contents of K^＋ , Na^＋ and Cl^- were measured by using the three analysis systems according to the sample sequence 1 to 8 and 8 to 1. In five days the results of 40 case samples were recorded and the deflective data were eliminated, the deflection was also estimate. Results： There was a better correlation among the three methods of electrolyte assay and the correlation modules （r） was 0.982 ～ 0.999. In the different points of medical decision standard, the expectant deflection did not exceed the half of allow able error of CLIA ＇88. Conclusion： The assay results of serum electrolytes （K^＋ , Na^＋ , Cl^- ） by using Olympus Au-2700 system were basically consistent with those by using Johnson Vitros-350 and XD-685 systems. Method comparation and bias estimation for judgement of clinical acceptability were necessary in order to insure the comparability of experimental results in a laboratory.
