摘 要:目的以小鼠为模型,探讨雌激素诱发白假丝酵母菌性阴道炎的免疫学发病机制,观察阴道内应用消炎痛对白假丝酵母菌性阴道炎的治疗效果,并分析其作用机制。方法设雌激素化小鼠白假丝酵母菌性阴道炎模型组(EI)和模型用药(消炎痛)组(Indo),另设单一白假丝酵母菌感染组(I)、单一雌激素化组(E)和空白对照组(C)。在接种后4、7、14d动态观察小鼠阴道灌洗液中白假丝酵母菌菌丝生长情况,并进行阴道组织病理学观察。分别采用免疫组织化学法及酶联免疫吸附(ELISA)法检测小鼠阴道组织TGF-β1、IL-2水平。结果Indo组小鼠未见菌丝,仅见孢子,EI组见大量菌丝生长。雌激素可上调小鼠阴道组织TGF-β1表达及下调IL-2水平(E组比C组,均P<0.05),EI组和I组的TGF-β1水平显著高于C组(均P<0.01),而白假丝酵母菌感染后IL-2水平未见升高(均P>0.05)。与EI组比较,Indo组在第4、7天阴道组织TGF-β1水平明显降低(均P<0.01),IL-2水平明显增高(分别为P<0.05,P<0.01)。结论雌激素能降低小鼠阴道局部免疫能力,白假丝酵母菌感染时小鼠阴道缺乏防护性的细胞免疫反应;消炎痛具有既能抑制白假丝酵母菌菌丝生长,又能调节阴道局部抗白假丝酵母菌感染免疫功能的双重作用,有望成为一种治疗白假丝酵母菌性阴道炎的新靶向药物。Objective To investigate the immunopathogenesis of vulvovaginal candidiasis induced by estrogen, and to study the effect of indomethacin on a murine model of vaginal candidiasis and the mechanism of its action. Methods The estrogentreated murine model of vaginal candidiasis(EI) was established. The groups of infected but non-estrogen-treated mice(I), estrogen-treated but uninfected mice(E), indomethacin-treated mice(Indo) and naive mice(C) were set up. The hypha in the vaginal lavage fluid and the pathology in the vaginal tissues were observed at day 4, 7 and 14 after the inoculation. Immunohistochemistry and enzyme-linked immunosorbent assay(ELISA) were applied to determine the transforming growth factor-β1 (TGF- β1) levels and the interleukin-2(IL-2) levelsin the vaginal tissues, respectively. Results In the vaginal lavage fluid and the vaginal mucosa, no hypha but spores were seen in Indo group and a great number of the hypha were seen in EI group. Estrogen up-regulated the level of TGF-βI and down-regulated the level of IL-2 in the murine vaginal tissues(E v C groups, both P〈 0.05). The levels of TGF-β1 in EI and I groups were higher than those in C group(both P〈0.01), but the levels of IL-2 in EI and I groups had no difference from those in C group(both P〉0.05). As compared with EI group, the levels of TGF-β1 were lower at day 4 and 7 after the inoculation (both P〈0.01), while the levels of IL-2 were higher at the same intervals(P〈0.05, P〈0.01) in Indo group. Conclusion Estrogen may depress the local immune function in murine vagina and on the onset of vaginal candidiasis, the vagina defect protective immunoreaction. Indomethacin has the double effects of inhibiting the hypha growth and regulating the vaginal local anti-infection immunoreaction in the murine model. So indomethacin can be a new targeted drug to the treatment of vaginal candidiasis.
