乙肝相关性肝硬化、肝癌患者外周血淋巴细胞Fas及其mRNA的表达

摘　　要：目的探讨乙型肝炎(简称乙肝)相关性肝硬化和肝癌患者外周血淋巴细胞Fas及FasmRNA表达水平以及血清中可溶性Fas(sFas)水平与疾病发生的关系。方法选择临床确诊的慢性乙肝患者20例、乙肝相关性肝硬化患者28例、HBV相关性肝癌患者20例以及健康对照者20例,分别采用流式细胞术和RT-PCR检测外周血淋巴细胞Fas及其mRNA水平,采用ELISA双抗体夹心法检测血清中sFas的水平。结果以上各组外周血淋巴细胞Fas水平分别为(49.6±10.8)%、(43.0±9.1)%、(70.4±7.02)%和(59.9±4.02)%,Fas mRNA表达水平分别为1.22±0.19、1.20±0.16、1.74±0.17和1.50±0.10,肝癌组外周血淋巴细胞Fas及其mRNA表达水平均明显高于对照组(P<0.05),而慢性乙肝和乙肝相关性肝硬化组均明显低于对照组(P<0.05)。各组血清sFas水平分别为(2.06±0.98)μg/L、(3.18±1.60)μg/L、(2.23±1.12)μg/L和(1.11±0.49)μg/L,与对照组比较,其他三组均明显升高(P<0.05)。结论外周血淋巴细胞Fas及其mRNA水平的高低和血清中sFas水平的变化可能与乙肝相关性肝硬化、肝癌的发生有一定的关系。Objective To investigate expression of Fas and Fas mRNA in the peripheral blood lymphocyte and serum soluble Fas （sFas） levels in patients with hepatitis B and HBV-related cirrhosis and hepatocellular carcinoma （HCC）. Methods Twenty patients with hepatitis B, 28 patients with HBV-related cirrhosis, 20 patients with HCC and 20 healthy subjects were included in the study. The expression of Fas and Fas mRNA in peripheral blood lymphocyte were detected by flow cytometry and RT-PCR and serum sFas levels were measured by ELISA. Results The expression of Fas and Fas mRNA in patients with hepatitis B, cirrhosis, HCC and normal controls were （49.6±10.8）, （43.0±9.1 ）, （ 70.4±7.02 ）, （ 59.9±4.02 ） %, and 1.22±0.19,1.20±0.16,1.74±0.17,1.50±0.10, respectively. Compared to normal controls, the expression level of Fas and Fas mRNA in HCC were significantly increased （P〈0.05）, however that in hepatitis B and HBV-related cirrhosis decreased （P〈0.05）. Serum sFas levels in above 4 groups were （2.06±0.98 ）, （3.18±1.60）, （2.23±1.12） and （ 1.11±0.49） μg/L, respectively. Conclusion The expression of Fas and Fas mRNA in peripheral blood lymphocyte and serum sFas levels may be associated with the development of HBV related cirrhosis and HCC.
