曲马多术前肌注用于眼科局麻手术镇痛的研究

摘　　要：目的 探讨曲马多（Tramadol）在眼科手术的辅助麻醉作用、术后镇痛效果和其恰当的术前用药时间。方法 我科2006年9月～2007年4月的住院局部麻醉手术113例，男59例，女54例。年龄15～78岁，平均41．37岁。随机分为4组：按术前用药（肌注）时间分为术前10min组、术前20min组、术前30min组（每组28例）和对照组（未用药，29例）。观察各组患者手术中不同时间段及手术后不同时间段的疼痛情况。按照疼痛4级分类法得出疼痛分级并进行比较。结果 用药组与对照组比较，疼痛等级均值在局麻（球后麻醉注药过程中）中、手术中及手术后均明显降低（P〈0．05）。结论 曲马多术前肌肉注射可在眼科局麻术中起到辅助麻醉作用，术后起到镇痛作用，并可缓解紧张、恐惧等情绪反应；起效时间在肌注后20min左右，作用高峰时间为肌注后1～1．5h，作用维持时间3～6h。Objective To investigate the assistance anesthesia and postoperative analgesia in ophthalmology local anesthesia operation with Tramadol preoperative injection. Methods 113 paients （59 men and 54 women ） from Sept. 2006Apr. 2007, age from 15 to 78 years , mean 41.37 years. They were divided into four group according to the time of injection： 10, 20, 30 minutes before operation and control group（ not injecting Tramadol Observing eyes＇ache during operation and postoperation . Working out pain grade and comparing with them according to Four grade grouping. Results The mean of pain grade of the drug group degraded obvi- ously during anesthesia, operation and postoperation comparing with the control group （ P 〈 0.05 ）. Conclusions This study certified that preoperative injection of Tramadol can produce effective analgesia in ophthalmology local anesthesia during and after operation. , and relieve patients＇ nevious and fearful moods The acting time of tramadol was about twenty minutes after intramuscular injection and the strongest time was 1 - 1.5 h and the lasting time was 3 - 6 h.
