摘  要:目的:近年来很多学者尝试应用药物干预扩张后皮瓣纤维包膜的形成过程,但在药物的选择上均不理想。观察几丁糖应用前后纤维包膜中胶原纤维含量及扩张后皮瓣回缩率的变化规律,分析几丁糖对纤维包膜中胶原含量的影响。方法:实验于2006-03/11在河北省人民医院临床研究中心完成,动物实验方法符合动物伦理学要求。①实验材料及分组:选用新西兰大白兔15只,每只背部中段脊柱两侧旁开3cm处设为扩张区,均将左侧设为几丁糖组,右侧设为对照组。②实验方法:几丁糖组在腔隙内注入20g/L医用几丁糖0.5mL,再植入20mL皮肤软组织扩张器1枚;对照组常规植入20mL皮肤软组织扩张器1枚。待实验动物切口愈合拆线后开始扩张注水,每只皮肤软组织扩张器注水达到40mL后停止注水。③实验评估:于停止注水10d后,切取两组扩张器表面相同部位纤维包膜及带纤维包膜皮瓣,用图像分析法计算各组织块收缩率;取纤维包膜组织作常规苏木精-伊红染色、Masson染色进行光镜观察,并应用真彩病理图像分析系统测量平均灰度,对比两组之间的差异。结果:实验设置几丁糖组和对照组各15例,全部取材成功。①几丁糖组纤维包膜组织平均灰度值显著高于对照组(170.383±14.313,159.762±10.623,P<0.05)。②几丁糖组扩张后皮瓣回缩率显著低于对照组[(43.371±7.312)%,(45.637±6.613)%(P<0.05)]。结论:应用几丁糖干预纤维包膜的形成过程,可以使纤维包膜中胶原含量降低,从而可以有效减少扩张后皮瓣的回缩率。AIM: In recent years, many researchers try to intervene the expanded skin flap by medicine, but medicine selection is disturbing them. This study observed the change rules of the content of collagen fiber in the fibrous capsule and the shrinkage rate of expanded skin flap by using chitosan, so as to explore the influence of chitosan on the content of collagen in fibrous capsule. METHODS: The experiment was performed at the Clinical Research Center of Hebei Provincial People's Hospital from March to November 2006. The experimental procedure was accorded with the animal ethical standards. (1)Fifteen New Zealand White rabbits were selected. Each animal was made two 3 cm incisions on both sides of back on the deep layer of subcutaneous superficial fascia. The left side served as chitosan group and the fight as control group. (2)0.5 mL (20 g/L) medical chitosan was injected into the laeunes of chitosan group and a 20 mL skin soft-tissue expander was implanted s, snbsequently. The control group was only implanted a 20 mL skin soft-tissue expander conventionally. When the wound healed, water was injected into the expander until 40 mL(3)Ten days post-injection, the fibrous capsule around the expander and the skin flap with fibrous capsule at the same site of two groups were harvested. The shrinkage rate was calculated using image analysis method. The fibrous capsule was taken to make HE staining and Masson staining slices and observed under light microscope. The ntean gray scale was measured with pathology image analysis system. The difference between the two groups was observed. RESULTS: Fifteen animals in each group were involved in the result analysis. (1)The mean gray scale of chitosan group was significantly higher than that in control group (1 70.383 ± 14.313, 159.762 ± 10.623. P 〈 0.05). (2)The skin flap shrinkage of chitosan group was significantly lower than that in control group [(43.371 ± 7.312)%, C5.637 ± 6.613)%, P 〈 0.051. CONCLUSION: Chitosan can reduce t
