肥胖症患者胆囊体积和收缩功能的超声研究

摘　　要：目的 探讨肥胖症患者与正常体质量者空腹胆囊体积及胆囊收缩功能的差异。方法 对44例肥胖症患者（BMI≥28kg／m^2）、47例体质量正常者（BMI＜24kg／m^2）行胆囊超声检查。分别于空腹状态下及脂餐后第30min、第45min、第60min、第120min测量胆囊最大长径、左右径、前后径，根据椭圆体公式计算空腹胆囊体积、餐后残余胆囊体积，计算胆囊排空率。比较肥胖组与正常体质量组之间空腹胆囊体积、脂餐后不同时段胆囊排空率的差异。结果 肥胖组的空腹胆囊体积明显大于正常对照组。脂餐后第30min、45min、60min胆囊排空率均明显小于对照组，以第45min差异最显著。餐后第120min与第60min的胆囊残余体积在两组内差异均无统计学意义。结论 肥胖症患者空腹胆囊体积增大，餐后收缩功能减弱。超声对于肥胖症患者胆囊收缩功能的评价时间可缩短至餐后60min，第45min为最佳观察时机。Objective To assess fasting gallbladder volumne and gallbladder contractility in obese patients. Methods Forty-four obese （BMI ≥ 28 kg/m^2 ） patients and 47 nonobese subjects （BMI〈 24 kg/m^2） were examined by real-time uhrasonography. The length, depth, and the width of the gallbladder were assessed after overnight fasting and 30 minutes,45 minutes,60 minutes, 120 minutes after fatty meal separately,the fasting volume（FV） and residual volume（RV） were calculated. The gallbladder ejection fraction was calculated with the formula： （FV- RV）/FV ×100%. The fasting gallbladder volume and gallbladder ejection fraction in obese patients were compared with nonobese subjects. Results Gallbladder fasting volume was larger in obese patients than in controls. The gallbladder ejection fractions at 30 min,45 min,60 min after fatty meal were all significantly greater in obese patients than in controls, especially at 45 min. The gallbladder residual volume at 120 min after meal was similar with that at 60 min in two groups. Conclusions Increased fasting volume and impaired contractility of the gallbladder were found in obese patients. The interval for ultrasonography to discriminate gallbladder emptying between obese and normal subjects can be shortened till 60 min after the fatty meal,and 45 min after meal is the best chance.
