雷米芬太尼抑制扩宫颈体动反应的半数有效浓度

摘　　要：目的测定复合靶控输注丙泊酚时雷米芬太尼抑制人工流产术中扩宫颈时体动反应的半数有效效应室靶浓度。方法40例择期行人工流产术的早孕病人，同时靶控输注丙泊酚和雷米芬太尼，固定丙泊酚的效应室浓度（3μg/ml），雷米芬太尼按序贯法给予。相邻效应室靶浓度之间比率为1．2。输注3min后开始手术。记录病人有无体动反应。结果抑制人工流产术扩宫颈时体动反应的雷米芬太尼Ce50为2．24ng／ml，95％可信区间为1．97～2．55ng／ml。结论在复合靶控输注3μg／ml丙泊酚时，雷米芬太尼抑制人工流产术扩宫颈的体动反应的半数有效效应室靶浓度为2．24ng／ml。Objective To determine the half-effective target effect-site concentration （Ce50） of remifentanil combined with TCI propofol to inhibit movement during dilatation and curettage. Methods Forty patients with selective dilatation and curettage operations were enrolled. TCI of remifentanil and propofol was performed simultaneously. Propofol concentration was fixed at 3 μg/ml. Remifentanil was given by up and down method and the ratio of two close effect concentrations was 1.2. Results The Ce50 of remifentanil was 2.24 ng/ml. The 95% confidence level was 1.97-2.55 ng/ml. Conclusion When combined with TCI propofol of 3 μg/ml, the half-effective target effect concentration of remifentanil to inhibit movement in dilatation and curettage is 2.24 ng/ml.
