摘 要:目的探讨系统性红斑狼疮继发干燥综合征(SLE-SSS)患者的^(99)Tc^mO_4^- 唾液腺动态显像特点。方法对42例 SLE-SSS 患者与10名健康对照者行常规^(99)Tc^mO_4^-唾液腺动态显像,获得并比较腮腺与颌下腺的时间一放射性曲线(TAC)及功能参数:15 min 摄锝率(UR)与酸反应泌锝率(SR)。结果在 SLE-SSS 组,21.4%的腮腺与59.5%(11.9%为单侧)的颌下腺 TAC 明显异常。SLE-SSS 组的 UR 与 SR 均值均低于对照组(P<0.01),且左侧腮腺 UR 均值低于右侧(P<0.01)。SLE-SSS 组腮腺的 UR 及 SR 分别与颌下腺的 UR 及 SR 呈明显正相关(P<0.05),腮腺及右侧颌下腺的 UR 与 SR 无明显相关性(P>0.05),左侧颌下腺的 UR 与 SR 呈明显正相关(P<0.01和<0.05)。结论 SLE-SSS 时腮腺与颌下腺的摄取及分泌功能受损程度呈正相关,且颌下腺受损程度重于腮腺;腺体自身摄取与分泌功能受损程度无必然关联,且双侧腺体的功能受损程度不对称。Objective Dynamic salivary scintigraphy with 99^Tc^m-pertechnetate was used in the current study to assess the functional characteristics of salivary glands in patients with Sjogren's syndrome (SSS) secondary to systemic lupus erythematosus (SLE). Methods Forty-two patients with SLE-SSS and 10 controls underwent the dynamic salivary scintigraphy with 99^Tc^m-pertechnetate. The time-activity curves (TACs) and functional parameters of parotid and submaxillary glands, including 99^Tc^m uptake ratio (UR) at 15 min and secretion ratio (SR) after citrate stimulus were obtained. Results In SLE-SSS group,TACs were obviously abnormal in 21.4% parotid gland and 59.5% ( 11.9% being unilateral) submaxillary gland. The UR and SR of SLE-SSS group were significant lower than those of controls ( P 〈 0.01 ), while mean UR of the left parotid gland was lower than that of the right ( P 〈 0.01 ). In SLE-SSS group, UR and SR of parotid gland positively correlated with those of submaxillary gland ( P 〈 0.05 ). The UR of parotid gland and right submaxillary gland did not correlate significantly with their SR ( P 〉 0.05 ), but the UR of left submaxillary gland positively correlated with its SR (P 〈 0.05). Conclusions In SLE-SSS, the damage on uptake and secretion function of parotid and submaxillary glands were related, but the later more severe. No correlation was found between the uptake and secretion in single gland, and the functional damage to bilateral glands might be asymmetric.
